Woukd you transfer a high level mosaic embryo?

Just got my pgt-a results back. Had 3 embryos sent for testing, 2 abnormal, 1 high level mosaic.

I feel very deflated and cried lots today.
We have 1 more cycle as we used access fertility 2 cycles package. I'm tempted to just move on to cycle 2.

The embryologist suggested in case we are thinking of transferring the mosaic, to have genetic counselling first. Maybe I'll do that. We also have our follow up consultation next Tuesday.
My concern really is if a pregnancy does occur, that chances of birth defects are higher...

I am 2 months from turning 40. AMH at the start of the year was 7.2. I had an AFC of 10, 8 eggs collected, 7 mature, 5 fertilised and 3 blastocyst (none normal).

What would you do?
Anyone transferred a high level mosaic embryo?

If you had the option to go on to cycle 2 but keep the mosaic embryo transferred I’d do that? That way you can always come back if that’s your only option

Clocloxx · 07/10/2024 09:42

I've read that mosaic embryos can develop into perfectly healthy babies. I once had a 5CC day 6 embryo that my clinic threw out but I hear of other people getting success from a 5cc, it was the only embryo i ever made with all my ivf cycles, honestly I think it's all chance your not Guaranteed a successful pregnancy with a top quality embryo either it's all chance!

Thanks both.
I emailed access fertility to see if they would allow us to keep the mosaic embryo in case we don't get any good embryos with the 2nd cycle. Waiting to see what they say... if they say we can't keep it and we have to transfer or lose it, we will probably transfer.
That said we want to see what our doctor and genetic counsellor say too.

@Clocloxx sorry to hear they discarded your embryo. Did they test it and it was abnormal?
What did you end up doing? Are you continuing with more cycles?

I've read that omnitrope (hcg) might help improve egg quality/euploidy so I will ask my consultant about that too

Hi @bubu24 , I've just found myself in a similar situation. It's such a difficult choice! I had 2 embryos tested, 1 is a high level mosaic embryo and the other one is 'no result'. So now we have to decide if we want to have the no result embryo retested and then decide on what to do with the high level mosaic one... We have another egg collection next week and we'll hopefully have more embryos, but if we don't get any euploid, I think we'll try the mosaic. Reading about it online, sounds like it's not likely to work but it'd be a shame to not give it a chance. It'd be interesting to hear if anyone has had luck with one of these!

bubu24 · 08/11/2024 21:24

Hi @BelleCrescent sorry to hear about your results. I would have been equally frustrated about the "no result" one. Are you thinking of asking them to retest it again? I think they have to do it free of charge for any no results.
It's a tough one with the high level mosaic. Did you have the genetic counselling yet? I found that very helpful as they go into the specifics and explain how much is affected, what could happen etc. The counsellor said the same as you read online, that most likely it won't take or you'd miscarry. And if it does implant and results in a pregnancy they would strongly recommend an amniocentesis at 12 weeks (in our case anyway). If the abnormality is there then it would be heart-breaking having to make the choice whether to terminate the pregnancy or not. So on balance we decided not to transfer and we moved on to cycle 2.

I'm on day 5 of stims so looking at egg collection potentially for next Sunday. They moved me to long protocol this cycle.
When is your egg collection next week? Keep me posted on how you get on. Keeping my fingers crossed for you and hope this round comes with some euploid embryos!

Hi @bubu24 and @BelleCrescent i also had non-ideal pgta results - one no result, one segmental aneuploid and one segmental mosaic. My clinic said the chances for the mosaic is 45% and the segmental is 25% which is not bad. They said chances of any defects carrying through to birth is 1% - super unlikely. There’s a 1000 mosaic study which you can find online which supports this. I definitely think it’s worth a go and just had my mosaic put in today. Best of luck to you both!!

bubu24 · 04/12/2024 20:31

@SamVan congrats on your transfer! I have read many women go on to give birth to perfectly healthy babies from mosaic embryos and especially if they're low mosaic they're almost as good as euploids. Hope you'll update us soon with a positive pregnancy result :)

I ended up with 2 euploids on my second cycle so I'm very happy with that. Probably looking to do a transfer in Jan.

@bubu24 congrats! That’s a great result! Did you do anything different this time? My doc thinks it’s just a roll of the dice each time as we didn’t get as good results as we expected for my age. If this transfer (and the untested one) do not work I’ll be doing another round early next year too.

bubu24 · 04/12/2024 20:54

@SamVan yes this cycle it was a long protocol. I took buserelin injections for 11 days for down regulation and then 12 days of stims. They scanned me daily for the last 3 days and left my follicles get quite large (I had 3 at 20-21mm on trigger day). I had double the number of egg collected, which was a shock, double the number fertilised and reaching blast stage (although only 2 were day 5, and 4 were day 6). We had 2 euploids, the rest abnormal.

I also drank 2-3l of water, 1 litre of milk and ate lots of eggs and lean protein. I also upped the dose of coq10, gave up coffee and tried to limit stress (took time off work etc). Not sure if diet, supplements etc made any difference though. It was probably all down to the change in protocol. Or maybe as your doc says a roll of the dice and I got lucky this time round.

@bubu24 thanks for sharing ☺️ I’m really hoping if I have to have another round it’ll be a better outcome. Congrats again on your good results and all the best for your transfer!

bubu24 · 04/12/2024 21:08

Thanks @SamVan I hope you don't need to do another round and that this transfer is successful. Sending positive vibes your way 🤗
