Hi all, just looking for some tips. We have MFI, low sperm count - from what I remember everything else was normal. We’re both 34 (33 at time of collection).

We have had one egg collection that resulted in 4 blasts (sorry if that term is wrong).

  • Fresh transfer didn’t work (I came on my period during the 2WW and so my progesterone was increased)
  • First frozen transfer resulted in chemical pregnancy
  • Second frozen transfer resulted in negative test
  • One embryo didn’t survive the thaw.

Are there any questions you’d be asking or changes you’d be looking for going in to a second egg collection?

I would ask for:

  • dna defragmentation test for him
  • hysteroscopy/hycosy for you if you haven't had one yet
  • blood clotting disorder investigations
  • immunology
  • thyroid tests
  • pgt test your embryos if finances are available

I'm sorry for all your disappointment. Have you had a follow up with your clinic? What did they suggest? Personally I wouldn't go mad with additional tests off the back of what's happened so far. You got a good number of blasts and it's probable it was just bad luck none worked. It takes 3 transfers on average for a successful outcome. It sounds like your protocol worked in terms of getting you embryos, it was just the implantation element where it fell down. Robert Winston's 'the essential fertility guide' is a good pragmatic evidence based guide to fertility testing and treatment, highly recommend.

Thyroid testing would usually be done routinely before any fresh cycle as part of your blood work - you could check this with the clinic. PGTA testing is not recommended to improve chances of pregnancy, only to reduce chance of miscarriage and therefore usually recommended more for those who have experienced multiple miscarriages or are older and at much higher risk of this. At your age your egg quality should be good so I personally wouldn't use money on it. Immunology research is limited and poor quality, many consultants don't view it as a worthwhile exploration/treatment. Presumably you had ICSI anyway so there would be no value in a DNA fragmentation test. HyCoSy tests patency of your tubes so again as you're already having IVF I don't think that would be needed. A hysteroscopy and blood clotting tests could be worthwhile at some point if you continue to experience implantation failure but I personally would probably try an embryo from a different fresh cycle first. Just my thoughts from what we've been through and I've read. I'm a similar age to you, 1 child from IVF age 31 and currently two failed transfers age 34 for a sibling. Wishing you luck x
