embryo transfer on day 7 of progesterone??

So I’m freaking out slightly and my clinic is being SUPER unhelpful at reassuring me. Just looking for others experiences of FET. I’m doing a HRT medicated cycle for frozen blastocyst (day 5) embryo. My clinic have said to start progesterone and then the transfer is on 7th day of taking progesterone.

i have previously had a fresh, natural and medicated (that transfer was on 6th day for progesterone it was at a different clinic)

does anyone else’s clinic do this timing wise? My clinic have said that is protocol for everyone. My last medicated cycle the embryo implanted but I had a miscarriage at 7 wks unfortunately. Just worried about the timing.

I’m due to start 400mg pessaries twice daily and 25mg lubion daily for progesterone.

would really appreciate any/all views and others experiences to help me be slightly less stressed xx

All my cycles have been progesterone 5 days before ET and before transfer it's less progesterone, around half.

I guess if they gave you those instructions, I would follow them until confirming.

Hi @Ellenn did you manage to get any more info from your clinic?
From everything I know - for a day5/6/7 embryo, the transfer should take place on the 6th day. So 5 full days of progesterone, and transfer on the 6th. This makes sure your lining is in sync with your embryo. I would really push your clinic on this, as it's so important the timing is right to give the embryo the best chance possible xx

@Jellyfish1956 yes unfortunately our clinic protocol for HRT cycles for blastocysts is always on the 7th day of taking progesterone (with day 1 being first day of taking it). I’ve asked if I could do on day 6 but they’ve said no. Unfortunately it’s through nhs so very little I can do but trust that they use this protocol cause it works.

they said once your body reaches a certain threshold of progesterone it should be receptive to an embryo, I’m assuming that by making it later potentially more women might have implantation? Although since all my previous transfers have implanted it’s frustrating that they won’t allow me to do the same timing (I’m on almost identical drugs but extra lubion progesterone) 🤷‍♀️

@Ellenn I'm on same amount of progesterone as you, but transfer was on day 6 (so 5 full days of progesterone before the transfer). This was for a day 5 blastocyst. I've had 2 previous FETs with day 5 blasts and they were both done on day 6 as well.

Hi, I don’t know if this is helpful but I thought I’d share as I’m searching for similar experiences.

Im about to have the biopsy and trio of tests to find out why I have recurrent implantation failure- the EMA, ALICE and ERA. One of the things they’re testing for relates to the timing of implantation- to find out which day of progesterone my lining is receptive. I could be pre-receptive and need an extra day, receptive - meaning timing is perfect or post receptive- meaning I have gone too far and would be better having a day less.

I’ve had 5 embryos transferred previously- all on day 6 of taking progesterone. That is with Care Fertility and my understanding is that they always do it on day 6 of progesterone unless you’ve had this biopsy that tells them different.

I have read that 2 out of 3 women will be receptive on day 6. This statistic is just what I’ve read off the internet though but my clinic has lead me to believe most women are receptive on day 6 of progesterone- that’s why they transfer on day 6 as standard. I’m wondering if I’m one of the women who might need an extra day and the test might show this. In which case my next transfer would be day 7 of progesterone. Incidentally, my lining always looks really good on scans and my bloods always show my progesterone levels are good. I’ve had top quality embryos transferred but nothing. This is why I’m having the biopsy.

I just write because I thought it was interesting that your clinic transfer on day 7 of progesterone as standard given that it is my understanding that day 7 isn’t the standard and the timing is so important that people have the biopsy to determine timing.

I hope that is helpful and not causing more stress. Perhaps you could discuss the biopsy with them? I’m glad I’m having it done now as it will hopefully give me some answers and make me feel more confident that they’re not just guessing - even if the tests shows I’m receptive timing wise and we stick to day 6. The consultant said I might just be unlucky but nothings wrong.

The tests also look at other things such as microbiom.

I had read about the tests a little so thanks for the info although generally for me it seems embryos have no bother with the implantation part it’s more getting past 6 wk stage 😢 so far I actually have a BFP from this transfer but still super early days for me … we will see if it ends up being viable
