Hi there!

IVF newbie and just wondering if anyone with a high AMH managed to do a fresh transfer? I've been told by my current clinic I should be OK and the plan is go ahead with a fresh transfer.

Has anyone experienced this?

Any advice would be greatly appreciated 🤍

Hi,

Yes it’s possible. I’ve had three rounds, first low response due to under medication as I had a very high AMH. Second did a freeze all as over 20 follicles on show and 18 eggs collected. (I hear 25 follicles is now cut off). Recent IVF and 14 eggs so had a fresh transfer.

Yes, over three egg collections I’ve had 14, 19 and 21 eggs retrieved (AMH of 30) and we’ve done a fresh transfer each time. Discomfort, but no OHSS.

Personally, I’d like to have done a freeze-all to give my body time to feel normal again before doing a transfer, but a freeze-all at my clinic cost an extra £1k that we didn’t have.

Although it is worth saying I was doing day 3 transfers - another couple of days recovery and a blastocyst transfer would’ve felt more comfortable!

Just out of interest how many fertilised? I know it only takes 1 but I'm just intrigued by the attrition rates.

@ThesisthenaViva - first round poor response, one day 3 embryo. Then of 18 eggs, three day 5 blastocysts. I had one failed FET, then twins from second round.

This round 14 eggs to 3 blasts.

So our blast rate was 15% first time and 21% second time. Follows broad rule of thumb (I’m not pushing the mini ivf just the math)

While there isn’t a 100% accuracy rate, as each case is different, we can break success rates down into commonly seen general percentages.

  • 80% of eggs retrieved should be mature
  • 80% of mature eggs should fertilize
  • 30-50% of fertilized embryos make it to the blastocyst/transfer stage
So say you just did your retrieval and they pulled 10 eggs. If 80% are mature that means there are 8 eggs to fertilize. Then if 80% of those properly fertilize that leaves us with 6 embryos. If the rate of embryos making it to blastocyst is 30-50% that leaves 1-3 embryos for transfer. These percentages are not exact for every person, though the rule of thumb is that 80-85% of eggs do not make it to the blastocyst stage.

@ThesisthenaViva The 3 day transfer at my clinic was a blanket rule for over 38s, but some clinics do it when the embryos aren't looking great quality - the thinking being that they're always better in the womb than in a lab (only roughly 30-50% of day 3s make it to blastocyst in the lab). I'd much prefer to have done blastocyst transfer though! We could've got through our cycles so much faster if we had hadn't done so many day 3 transfers (no baby yet so we're moving to donor eggs next)

My AMH was 74 when I had my ivf. We got 17 eggs and by day 5 only one decent embryo to transfer. They transferred it and I did get OHSS pretty bad but it was resolved by the time I was 7 weeks (so successful cycle). Little boy is nearly 2 now and ttc a second

@ThesisthenaViva yes it's possible. My amh was 62 prior to ivf (pcos) I didn't retrieve that many eggs though as was put on a lose dose. I did a fresh transfer last Sunday. A fresh or frozen will probably be assessed depending on how your responding
