Another failed embryo transfer - any advice on what tests to ask for next time, treatment via NHS

I'm looking for a little advice. Just tested BFN after the third transfer and losing the will to live. We are lucky enough to have another 3 frosties but I'm reluctant to transfer if the same previous routines are followed.
Any advice on what/if any additional tests I can request before agreeing to the next transfer. I personally think there is more of an issue with myself than the embryos. My periods are extremely light only lasting a day or so, however, I do ovulate regularly. The last 2 transfers, my period came before my due date and also while taking suppositories (maybe progesterone is too low, any thoughts?) . I've also previously had prolactin issues which I'm now successfully being treated for so have 4 monthly bloods for this and thyroid, last bloods were done just before the last transfer and were apparently all OK.
I'm having treatment via the NHS so I'm not sure what additional tests they offer, if any at all. Would they automatically offer these tests after 3 transfers (first ended in an 8 week miscarriage, other 2 transfer failed to implant) or would I need to push for these tests.
If the NHS doesn't offer additional testing, am I able to request and pay for these tests? If so, any idea of costs.
Any advice would be much appreciated x

If your periods are light have you been checked for adhesions in the lining of the uterus? Ashermans?

I'm so sorry that it didn't work this time too. Please don't lose hope. Ask for a genetic testing on embryos. This way you can get rid of all defective embryos which massively increases your chances. Big hugs 🤗

@SH998 Did they test your progesterone before transfer? I would ask for that to be tested and if it is too low then ask for injectable progesterone. Unfortunately the pessaries do not absorb well for some people. From memory a private blood test for this is around £50 although the injections themselves are quite expensive if you have to pay for those privately.

Thank you all for commenting, its been extremely helpful. I've spoken to them today and the Dr was great. He agreed instantly to up my progesterone, put me on blood thinners if needed and is also putting me down for a series of additional blood tests (hopefully this will be done in the next week or so). He did mention a hysteroscopy but there is a waiting list of approx. 6/7 months I didn't want to wait that long to proceed with the transfers due to age but I still might ask to be put on the list as maybe a cancellation will come up.
I already feel better knowing that I've been listened too and more is being done :) x

Hi lovely, how did you get on? i feel like i am in the same position and waiting on a hysteroscopy via NHS.

SH998 · 02/06/2023 11:14

@kjdk how are you managing, is it a long wait? Where are you in your treatment?
I opted against the hysteroscopy due to the NHS waiting time and also my age (nearly 40). They wouldn't allow another transfer until that was complete if I was on the list and I wasn't prepared to hold off another 6/7 months. However, if this fails and I go private, its probably an additional test I will pay for.
I did however, have a series of additional blood tests which after 8 long weeks confirmed all was OK. I have never been so upset seeing an OK blood result, on this occasion I wanted something to show so hopefully it could be resolved. This week I've had my treatment plan so hoping to begin on my next period end of this month. They have increased my progesterone so hopefully that will work :)

@SH998 I bled before the OTD with my first fresh transfer, and had a BFN wjth a 5aa embryo. I pushed for lubion injections as I have borderline low progesterone anyway and took high dose vit D too for the first frozen transfer and that one stuck. Not sure if it's that but getting extra progesterone definitely helped me, pessaries just didn't seem to absorb. I took the injections til 12 weeks and placenta was formed so took over progesterone production.
I hope this transfer is the one for you! :)

@SH998 I hope everything goes well with the next transfer and that the adaptations made really work for you. However, I can really recommend Dr Thum at the Lister if you are wanting to look into private treatment. We went to him following recurrent miscarriage (however, repeated implantation failure is also one of his specialities) and he was so thorough and personable. We came away with answers that we never thought we would get and are currently going through our first NHS IVF cycle where we have pushed for adaptations based upon this. If you have any questions or if you would like some further information, please feel free to message me. I’ve got everything crossed for you x
