high AMH / low SHBG

Does anyone have experience of high AMH (for your age) or low SHBG?

I'm nervous to post after having to report a member's replies who attacked me for already having had successful pregnancies, yet the underlying challenge of subfertility seems to be the same now as it was for me when I was 29 with primary infertility investigations or aged 32 taking clomid for secondary infertility.

I'm 44 now and AMH came back at 32 pmol/L or 4.48 ng/mL, SHBG was low at 25 nmol/L. I checked my cortisol as well because of the SHBG and it is just out of normal range. My FSH was 5.24IU/L which is good and low and LH 6 IU/L.

Testosterone was slightly raised, but not really concerning and my blood sugar was really good, but my diet has been very low Gi for months.

I'm taking an educated guess that the AMH is why I've had irregular cycles for decades and why they are better now. I thought possibly estrogen dominance can drop with age too and read PCOS can partly reserve your fertility into your 40s, but that egg quality is still a big concern then. I've always been borderline or suspected PCOS, but never actually been diagnosed and whatever is underlying seems relatively well managed currently.

I'm not really sure about the SHBG other than it is probably raising cortisol and testosterone, but my body is still managing to balance things out as my cycles are pretty regular for me at 32-39 days now. I'm taking supplements to help with egg quality and progesterone, but also wary after having a MMC at 9-10 weeks in September and I'm sure I have issues with low progesterone. This is really the first time in about 10 years my cycles have been balanced enough to conceive, partly because I had a stint on the pill around 2017 when I was qualifying as a teacher and didn't want to risk having to deal with more miscarriages. That may have been unwise, but it was easier to shut everything down hormonally then.

I'm taking supplements and vitamins to increase egg quality and progesterone. The only things I think I can do is more walking to try to push cortisol down and starting to track estrogen and progesterone across the second half of my cycle with Mira. I thought of adding in a progesterone cream in luteal phase., but want to track progesterone levels through a cycle first.

I don't think any medics will give me progesterone suppositories if I have another pregnancy because of my age and I don't know if that is something I could get privately. They seem to assume your body is competent or not. IVF or IUI isn't something we would have ever done as we'd have better luck naturally (plus too old now), but it is really hard to get a positive test despite very good timing and then the risk of early losses, which I seem to just have to go through. The one in September was my fourth miscarriage. The miscarriage clinic don't want to see me because of my age and the fact I have had two successful pregnancies, so any testing I've had to do independently. They said if I had another miscarriage they might reconsider. I think because the previous losses were when I was 32. All my gut feelings are to keep trying though and that my blood tests reinforce this.
