Help please - what tests do I need to find out why I am not getting pregnant?

Thank you all in advance - I am confused.

I am 38, have had one pregnancy that was a TFMR. I have had two sets of blood tests with the GP that are “normal”.

I want to get further tests to understand if there is a problem, or if I have just been unlucky so far.

Ive read loads of threads about AMH, FSH and other things and it’s just not plain enough for me - please can someone explain what tests I need to understand if I have any eggs left and if they are any good?

GP keeps telling me to relax and it will be fine. I don’t feel confident in that aged 38.

With AMH, if it is low then likely to be age-related and therefore low-quality eggs or if FSH (repeated) is at any time above 10-12 units similarly. Elevated FSH indicates diminished ovarian reserve. If your GP has tested those and says they are normal that is probably a good sign.

I am waiting for luteal phase progesterone test (7 days after you think you've ovulated you test progesterone levels to confirm as it peaks then), then will do day 2-5 female fertility bloods myself with Medichecks, which I ordered in advance. So female fertility blood tests, AMH, Iron, DHEA and CRP (which is for me to check for inflammation).

I had suspected PCOS previously so estrogen dominance and insulin resistance. I think my cycles are regulating more now as estrogen drops as you get older (I am 45). I want to test FSH as AMH as these should give me a good indication of what is going on, but I still think I have issues with progesterone deficiency.

Beyond that, more in-depth tests might be Antiphospholipid syndrome tests, Full thrombophilia profile and karyotyping of yourself and partner. They are more expensive and I'm not sure they would be worth the while, unless they obviously picked something up. Those might pick up reasons why a women can't carry a pregnancy or why there might be more likelihood of genetic abnormalities like unbalanced translocation.

Thank you so much something at a quick scan that seems much clearer, appreciate you taking the time to reply.

I want to get as much information as I can about what is happening in my body and whether there are problems, but did not know what I was asking for and kept being told that it would just be fine in time. Usually by people who have children and had no problems :(

Also your partner needs a semen analysis too!
