Poor sperm, previous testicular surgery-any hope?

Hello everyone, hoping anyone in a similar situation can give advice? My husband had testicular surgery as a child for suspected torsion. Initial sperm analysis showed infection in the sperm, low motility and morphology and count ok. Given antibiotics and infection normalised but other parameters still low. Tried co enzyme Q10 Wellman conception acetyl cisteine but no improvement and in fact count is now low too! 3 cycles of icsi have all resulted in poor quality embryos although we get good fertilisation rates.
Just started proxeed.

Our consultant feels there is not much that can be done to improve the sperm as this is likely all related to his childhood surgery. Has anyone been in a similar situation? Was there anything that helped?
He doesn't smoke or drink, normal weight, testicular scan normal. I'm trying to convince him to do DNA fragmentation/see a urologist/try something like IMSI or PICSI but the impression from our consultant is it's not going to help. 5 years TTC, never been pregnant, I have regular periods, so hard to have any hope :(

My husband has low sperm count because of a varicocele that was missed by the gp ten years ago. An attempt to embolize it to improve things failed as the surgeon couldn't find the right vein, so we're going down IVF with ICSI route, in full awareness we may have embryo drop offs due to dna fragmentation due to the varicocele. Hoping that because my tests have all come back fine my eggs are healthy and ICSI will be the magic we need to become parents, hope you find a route to parenthood too!

Hi @Natw22 no advice as such but I'm in a really similar position. My parter has very low motility and also low morphology, count is ok. He had surgery as a child for torsion and also has a small varicose now. No issues on my end, good AMH, AFC, regular periods etc.

Weve had two failed rounds of ICSI, which was 3 transfers. First chemical, second MC and most recent negative test. My NHS clinic are of the view that we should just push on with a third round but I don't want to without further testing for my partner, which they aren't willing to do.

We have booked to see Mr Ramsey in London for a DNA test and hopfully he can help us before we do a third round of ICSI and we are also going to change clinics just for a fresh pair of eyes so to speak.

Its a lot of money to see Mr Ramsey, transport, hotel etc along has cost hundreds as we live up North! But, i feel like it'll give us the best chance at another round, or at the very least we can see at least we tried everything we could.

@dogmam have your clinics not offered surgical sperm removal? I red that circumvents dna fragmentation ?

@Imisscoffee2021 no, my clinic would only do surgical sperm removal if there were no sperm at all. They also don't do any tests etc for dna frag and don't really believe in it which is why I've got to go private for the tests elsewhere.

Hey how did you get on? We’ve just been told that it’s best to see a urologist and not to go ahead with icsi atm as sperm count is low so chances of a successful result is low also..

did you partner see the urologist? what can you expect? Thanks
