Day 3 bloods normal, but PCOS?

I was diagnosed with PCOS as a teenager with very infrequent periods (following ultrasound and bloods).

Fast forward to coming off the pill many years later to TTC and my cycles have been getting longer and longer, which didn't come as a particular surprise.

I just had day 3 bloods done (after almost six months since my last period) and was expecting them to re-confirm PCOS, but I've had a text from the doctor saying all is normal apart from an iron deficiency, so I should take medication and then test again in 3 months.

I'm a bit stumped - I just assumed it was the PCOS and was gearing up to start tackling that, e.g. with Clomid. I've made an appointment to discuss where to go from here, but does anyone have any thoughts? I'm guessing iron deficiency isn't the cause (although it's not the first time I've had it flagged up in a blood test). Thank you for any thoughts!

Ah now a bit concerned I messed up the timing of the test. My periods tend to be super light and short, so when I saw a hint of blood on Sunday night I scrambled to get a blood test for Wednesday. But now (Thursday, Friday and through to today) I only just seem to be in full flow - the other days it was more like spotting in comparison.

Pretty annoyed at myself if that will make a difference. There’s just such a lack of information, though. Feeling a bit rubbish today.

You need day 3 and day 21 bloods @fungibletoken . The day 21 bloods would show no ovulation if you have long cycles, which is one PCOS indicator. PCOS diagnosis requires 2/3 symptoms out of: long/irregular cycles, cysts on ovaries, hormone profile unusual. Day 3 bloods can only look at the LH/FSH hormone profile which may not be out of normal.

Also, I'd ask GP to give you the actual values. GPs know v little about fertility in my experience. They have told me before that bloods were normal when they were not. Did they do androgens too eg testosterone, progesterone, prolactin, androstenedione?

Be prepared that in my healthboard a GP cannot give you clomid or letrozole. It's the fertility clinic that needs to do that and they will only take you after 1 year TTC. I'd say it's important to be under care of fertility clinic as they have to titrate the dose with regular scans and bloods and a dose being too high is dangerous for your health and fertility. I wouldn't want to do clomid or letrozole without fertility clinic monitoring
