Implantation issue

What can the clinic do to assist with implantation failure.

Every test result is optimal. Our age everything. Embryos created through ivf are top quality but it's failing.

I'm losing hope

Hi @1940s can you tell us your age, what treatment you've had to date (fresh cycle results, FETS attempted etc), what tests you've had to date and if any issues have arisen during treatment.

I'm 33 had a fresh and a frozen. Tried naturally for 18 months. One failed IUI.

My biggest concern is that this is unexplained secondary infertility and I had a c section

All I can logically understand is my uterus is now 'broken'

No issues during treatments. No adverse or strange effects. 12 eggs collected resulting in 5 blasts, one used fresh the rest frozen

What has gone wrong from first easily conceived child (born 3 years ago in June 2018) it must be the c section but all scans have said scarring is 'ok'

To be honest my c section was the cause of my secondary infertility as my tubes never really worked again afterwards and I ended up losing both to ectopics

I guess you could also look at PGS testing to see if there are any chromosome issues going on but the science isn't 100% accurate

You could also look at having NK cells testing done but if your clinic puts you on prednisolone anyway as part of your transfer protocol then that should cover it

The stats show that most people require at least 3 transfers to have success as there is roughly a 30% chance each time of it working at your age so the odds are in your favour for the next transfer ☺️

What grade of blastocysts did you manage to get frozen? X

So it may just be bad luck at this point. The chance of success with an untested embryo is around 1 in 4 to 1 in 3. Studies have shown that success rates stay pretty consistent even up to the 4th or 5th transfer, it’s only after this point that people start considering implantation failure.

I personally knew something was wrong in my case and went for a whole host of investigations after my 2nd unsuccessful transfer. I have never had a BFP before. I had excellent results from my fresh cycle, 12 eggs were retrieved, all mature, all fertilized and we got 9 blasts, half were the best possible grading. 83% were euploid. So it seemed to be a clear case of an issue with implantation. It ended up being that I had no good bacteria in my uterus, although after that was treated I had another unsuccessful transfer, this time with a euploid embryo. But it's never a certainty even with a euploid so that may have been bad luck.

@cream123 may be able to advise about the c-section scar.

Thanks so much for the guidance so far.

It's just so illogical that my body can fall pregnant so easily for child one and even meds and intervention can't make baby two happen (so far) it's hard to understand what's going to change. So many good friends of mine have needed IVF but it's because of a very specific obvious problem and then IVF has been the magic answer. I truly feel it's never going to work

@1940s I think it's reassuring that you've been able to have a child before. It is common for people to express what you have and I think in particular people who describe having no issue at all having the first, end up shocked that it is so difficult to have another. But I mean there is so much that has to line up perfectly to have a child and a lot is down to luck.

So many people going through IVF have unexplained infertility. I had that once, until I had a huge amount of tests and have now collected a lot of diagnoses! But these tests don't really make a difference in the early stages.

I think people seeing IVF as the "magic answer" is unrealistic. It has the ability to increase chances, and if someone has the ability generate embryos and keep having transfers then the cumulative chance of success increases.

Statistically 2 unsuccessful transfers really doesn't mean much. I wouldn't conclude that this is evidence that it won't work, because it's not what the evidence base shows.

Thanks Garden, it's hard to think rationally isn't it. If I look at the hard facts then I'd state 'that person' has a good chance of having another child. But in my head I'm doomed and will have only one child!

How quickly can I move onto my next transfer?

If I start bleeding today and today is CD1 can we do something in this cycle or will they make me wait again and if so why?! Surely they can do something approximately on day 14 of this new cycle straight from failed transfer?

@1940s if this is a withdrawal bleed after stopping medications, then most clinics will want you to have a natural cycle and the FET then starts following that. But it's down to your specialist, you should book in a review appointment to discuss if any changes need to be made for the next cycle.
