Why don't I ovulate?

Or have periods?

All my blood tests and scans have come back normal so what possible reason could there be? I am being referred to the fertility dept but just wondered if anyone had been in the same position and if there is anything I can do whilst I wait. I am so lucky to have 2dc already - first conceived with clomid, second one is a miracle. Originally they said it looked like I had a mild form of PCOS but in the latest blood tests theres no indication of that so who knows.... we have been ttc #3 for 3 years and during that time I've had 1 period and 1 mc. I guess its just clsssed as unexplained? But there MUST be a reason...

How old are you? When you say you don’t ovulate, how do you know? Usually ovulation is confirmed with a day 21 progesterone, but you said your bloods were normal, was this normal?

Have you had surgery before? I am being investigated for Ashermans syndrome at the moment, while I have never had surgery most women who get this condition have e.g a d and c. People with the condition will still ovulate but will have very light or nonexistent periods.

Hi @Gardenlady543 thanks for replying. I'm 32 and recently had day 21 bloods but wasnt on day 21 as I havent had a period (other than my mc in Feb) June 2020 so it was a bit of a stab in the dark but she said they didnt check for progesterone for some reason so that will need to be done. Other than that, I have no periods and my understanding is that a period always follows ovulation but I guess that may not be the case? I did get a positive opk about a month ago but no pregnancy or period followed. I constantly have ewcm which surely would indicate some imbalance somewhere...

@Fuzzyduck21 that’s good that you’re young, what is your AMH and AFC?

Ah yes I can see it will be difficult to time the day 21 progesterone with no indication of cycles.

In Ashermans syndrome there is scaring and adhesions in the uterus, which means periods are either light or non existent, but people will still ovulate. Did you have any surgery- d and c, c section etc? I am seeing a specialist tomorrow, I’ve had really light periods since I had the mirena coil removed.

@Gardenlady543 luckily didn't need a D&C and the miscarriage was completed naturally. No surgery at any other time either and have never had the coil. My periods have always been very irregular (1 or 2 a year) since they began around age 16. I was just fobbed off and put on the pill. It'll be interesting to see what the fertility people say whenever I end up seeing them.

Hope you get some answers. Is there anything they can do for you if it is that?

Sadly I didnt get to take down my numbers as I was out when the doctor called this morning but could ring and maybe the secretary could let me know?

@Fuzzyduck21 I only ask about the AMH and AFC because that will give some indication about your egg reserve. Also the AFC is useful when assessing for PCOS.

Oh that’s interesting that you’ve always had cycles like this. I would be highly suspicious about PCOS then, which you said was mentioned. The diagnostic criteria is

  • Polycystic Ovaries on Ultrasound (>= 12 Antral Follicles in One Ovary or Ovarian Volume >= 10 cm3)

Plus one of these:

  • Clinical Hyperandrogenism (Ferriman-Gallwey Score >8) or Biochemical Hyperandrogenism (Elevated Total/Free Testosterone)
  • Oligomenorrhea (Less Than 6-9 Menses per Year) or Oligo-Ovulation OR

You clearly have very few periods, so if you have a high AFC, that would seem to be the most likely explanation.

I agree, it will definitely be useful for you to see a fertility specialist about your case.
