Can anyone help me here?

I have had bloods taken and just had the results this morning. I'm trying. As I I have irregular cycles they have done a day 8 test which showed my LH level to be high

LH 20.3 (14 is top end normal)
Testosterone 0.7
TSH 2.14
Prolactin 389 (should be between 100-500)
FSH 9

What does all this mean? I'm already thinking it could be PCOS as I've read high LH is an indicator, and that 2:1 ratio with FSH is also a diagnostic!

I've been TTC 12 months and my periods range from 26 days to 38 days. I almost always get a positive OPK each month, which tends to average out right according to length of cycle (if that makes sense!)

Anyone else have similar? Any success stories?

I understand from most people's posts that LH and FSH are done on day 2. No idea why mine are done on day 8, only that the fertility clinic that I've been referred to recommend this for irregular cycles.

I appreciate any help and feeling very anxious. I have an appointment with my GP on 9th October, and first fertility clinic appointment on 16th October.

hmm it's a tricky one. LH and FSH are best tested on day 2, I'm not really sure what that's telling you on day 8.

was this a fertility clinic that advised day 8?

BewilderedBee · 23/09/2019 23:30

Thaanks for your reply @bubbaflash it was my GP but based on instructions from the fertility clinic. The clinics website also says CD8 to test irregular cycles. My test ended up being on CD9 though as CD8 was a Sunday. Got my positive OPK on CD14 so wondering if this is why it was high. But half thinking I'm making that up to make myself feel better!!

no it would probably explain why some of your levels are elevated. I'd be tempted to get them done again on cycle day 2. I've you know when you ovulated you should be able to book day 2 tests for anticipation of your period arriving. cycle day 2 or 3 would be fine

AttilaTheMeerkat · 24/09/2019 10:36

The NHS trust I was under used to do CD10 tests so this may be what they are thinking of along with day 2 and day 21 (all these gave a far more accurate picture of hormone levels).

You do need a diagnosis first and foremost. I would certainly ask them re the cycle days of the blood tests. With regards to this forthcoming appointment do not go on your own if at all possible and write down anything you want to ask well ahead of time.

If periods are irregular as well these tests can be done according to calendar days. Would refrain completely from using OPK; these are not recommended if the cycle is consistently irregular.

Somehow I deleted my reply!!

Anywho, I was going to say that the reason it's taken cd2-5 is because the range of 'normal' is wider at different points in your cycle.

www.urmc.rochester.edu/encyclopedia/content.aspx?contenttypeid=167&contentid=folliclestimulatinghormone

It's also agreed that the 1:2 ratio only holds cd 2-5 as well.

www.gloshospitals.nhs.uk/our-services/services-we-offer/pathology/tests-and-investigations/lh-and-fsh-gonadotrophins/

I'd ask the clinic to explain their reasoning for the cd 8 test, as I don't think this is 'usual'. Depending on their answer, not sure I'd trust them... What have they said about your results?? They, surely, should get first crack at interpretation instead of interested lay-women from the internet.

@AttilaTheMeerkat - I'm what way is the picture more accurate? The reference ranges I've only ever seen are narrowest (and therefor more diagnostic) during the early follicular phase.

Your cycles aren't 'so irregular' as to render the correct cd meaningless. Calendar days is more usually used with women who have cycles 50+ days, for example. Or women who have very few periods each year.

And unfortunately that link isn't working...
