FET cycle underway - progynova concern

Hi I've just started real cycle for FET (this is my 4th ivf cycle). My recent mock cycle threw up an issue with thin lining (never happened with previous cycles) and clinic (Spain) has upped Progynova daily dose from 6mg to 16mg!! 8 orally and 8 vaginally. Anyone else has this? They've also added daily Aspirin but my UK monitoring clinic medical director said Not to take Aspirin as he believes adversely affects implantation!! I'm conflicted!!!! Help if you can :)

I've had this exact same issue. You're not with IVF Spain are you? I am! I've just had a transfer with them this week.

I have followed the advice of the clinic in Spain, although concerning your my lining didn't get much thicker tbh despite all the extra Progynova.

Yes! Ivf Spain. Transfer date due 10th June. Never had lining issue before but so conflicted about Aspirin. We're you on such a high progynova dose??

That's a LOT of progynova. I had a lining issue and I was advised to start taking 12mg instead of 6mg - worked a treat, had a lovely healthy lining by transfer day. I wonder is it possible for the lining to be too thick? Or if it can only grow at a certain maximum rate, no matter how much you take? I think you should question your clinic further - you're paying them for treatment, they need to give you enough information so that you're confident in the medications you're taking.

I've taken aspirin and was given different advice by different clinics - one said to take it from day of egg collection onwards, the other advises starting on day of embryo transfer. I followed instructions each time and the cycle that worked is the one where I started on day of egg collection. But that might not be meaningful!

@TryingSpain yes. I was on 6mg in my mock cycle and my lining was around 7mm. They wanted it to be a bit more so in the real cycle they upped it to 8mg (all orally). However at my real cycle scan my lining was around 5.7-5.9 so they upped me to 16mg (8 orally 8 vaginally) and added the aspirin. I had another scan and I was only at 6.2ish. They were happy to go ahead with transfer anyway but added in viagra! and on the day of transfer I was more like 6.7.

They said the drop between the mock and real cycles could have been due to an injection I had - decapeptyl I think. Have you had this?

To be honest I just felt like I should trust what they were saying and go with it 🤷‍♀️

Yes @jemimafuddleduck I got the decapeptyl injection too. This is my 2nd FET so I got that injection last time around Nov 2017 but BFN with that cycle. I've never had lining issue before but to be honest I think that all of it is a bit of a lottery. For my first fresh transfer back in May 2017 I got a BFP but miscarried at 9 weeks. I didn't take the Aspirin that time as advised by my UK monitoring clinic. For the next cycle I had an Endo scratch and took all meds including the Aspirin and had BFN. My learning from it all is to be positive but not to rely on the absolutes of the science (there are no absolutes). I'm channelling my Hippy side and having Reiki, Reflexology and Mediating A LOT 😂

You HAVE to take a blood thinner if you take Progynova. It comes with a risk of Deep Vein Thrombosis

Thanks @redpandafluff I have asked the question but I would say that having treatment abroad there are definitely communication challenges. It shouldn't really be but the reality is that it is a wee bit harder - I have definitely felt a little adrift.

@TryingSpain I have felt the same. I have also felt that there have been A LOT of add on costs which I wasn't aware of at the time - one of which is the drugs they put you on post transfer which when I took the prescription to the pharmacy round the corner it came to 1400 Euro!!!!!! Firstly I had no idea that I would be put on extra meds and secondly the cost! I was really upset. Luckily a lot of it is cheaper in the UK (Progynova alone came to 994 euro) so I got only what I needed and will get the rest in the UK. They really aren't great at keeping you in the loop about your cycle.

@jemimafuddleduck I agree although I have to say I've not been impressed by any of the clinics I've attended. I genuinely have done some soul searching and I don't believe it's because my expectations are too high. I really think they don't truly understand the journey and some of the most simple things around communication can mean the biggest thing to the end user I. E. The patient. I've often thought why are there so many fertility and IVF forums like these? The experts tell you to stay away from them But they exist because there is a need! If you got the support, answers and info you really needed during the IVF journey no one would turn to these forums! I think it's really a shame because there is huge gap in what they think is their standard of patient care and the reality for those experiencing it.

I hear ya - two of my cycles were in Prague and I had to really push for clarity on some aspects. It's frustrating; not at all what you need at an already stressful time.

In the UK where it’s all regulated but so is a bit different. I was told progynova needs a blood thinner and progesterone on a medicated cycle because otherwise it causes blood clots. I had no womb issues but I was on 6mg (3 tablets) of progynova + 800mg of progesterone + aspirin (before transfer) / heparin (after transfer) throughout my medicated FET right through to 12’weeks.

That seems a huge dose imo

Yes a lining can get too thick - my clinic warned me about that as was taking for longer as on holiday

So we said I would be scanned and if lining too thickness then stop and start following month

But it was fine

I went to ivi in Madrid

And yes was on aspirin and daily haparin injections

Yep @blondeshavemorefun I thought huge dose! Scan to check lining tomorrow so we will see what effect it has had!
