Hi all.

Bit of background.

5 cycle (one DS on 4th cycle)

High responder...AMH is 57.

Last cycle - Simmed for 20 days, 38 eggs and OHSS.

This cycle, same clinic. I was put on a higher doseage as last cycle it took me longer to respond. On my day 5 scan (1st scan) it was lowered to 150iu. 3 days later and after only 7 days of stimms i was apparently ready for EC. I was shocked. I had lots of follies but from what i could see, measured 12-13mm.
Come EC yesterday, out of a shocking 70 follicles, they only retrieved 12 eggs. I aked the doctor how? Why was the number so low, she said had they left me til friday, i would have got too many. 🤔
Ive just received the call that only 2 of my 12 were mature and NONE fertilized. The embryologist even said herself that something wasnt right and they are shocked as i was.

Now, im gut is telling me that EC was too soon, they have massively f**ked up this cycle and i either want to be fully reimbursed or a free cycle.
What are my chances? Has this ever happened to anyone at around my age (34) with my AMH?

Im devastated. 😪😪😪😪

Also, its funny because this was my first cycle where i was on metformin and this obviously was my worst cycle yet! However, i am back taking it again ready for my next cycle next month

Metformin works because it has a supressing effect. It is supposed to, with medication, help create fewer but more mature eggs. Is it possible you need your dose reviewed?

Haha! Well, it did half the job! Just forgot to mature them!! 😂
My consultant put me on them to avoid OHSS and it does help with egg quality.

Yeah, i had my dose reviewed on my follow up.
So my 1st cycle with them (my 4th cycle in total) i was on 150iu, never responded so they gradually upped the dose until i was on 300iu, i started responding on day 13!
So this cycle, we started on 225iu... i responded straight away, by day 5 scan, my bloods were coming back what they expect to see at day 8 so it was lowered.
It was decided on my follow up that we would go back to the start and start on 150iu and take it slow again...
See where that takes us. Hopefully not as many eggs and not OHSS

Just a little update.

The clinic have had their meeting and discussed my case. They said they still have a little more information so she said by end of play on Monday, i should know the outcome.
Im not sure if this is positive or negative news, but i take some comfort in knowing they obviously havent dismissed it and are really looking into it.
Hopefully, being true to their word, i can update you on Monday

Hey Pass,

Fingers crossed for Monday...

Thinking about next time, did you talk about a Buserelin trigger at all? That all but eliminates the chances of OHSS so they don't have to be 'careful'

There may be a good reason they didn't use it but I'm surprised they didn't even discuss it with you. It worked for me - I started off with 45ish follicles, of which 30 grew to a decent size. From those I got 31 eggs, of which 27 were mature...

Hi outofmydepth.

Great minds! I did mention using an alternative but i couldnt remember the name and said frostimin.. (or however its spelt) and he just said no that frostimin is a stimulate drug not trigger, and that was that.
I may mention it to him. Once i know the outcome.
Im so nervous for tomorrow 😐

Nope, still no outcome. She emailed me yesterday to say that she was still waiting for more medical information and would catch up with the doctors today to get it.
This really makes me think its a no. I think they are waiting for this info as backup for their reasons to decline 😞
Hopefully today i will finally know

As thought, the clinic said no refund.

Im little miffed as my consultant lied and said i requested a higher dose in my initial consultation. Thus wast true 😒 he told me he would put me in a higher dose and when i picked up my prescription, it was the average dose and that is what i questioned...
My friend is telling me to fight it but she isnt from the IVF world. The clinic have made a few wrong decisions but as i have PCOS, i dont have a leg to stand on.
Now its time to decide whether to put ourselves in even more debt and stretch ourselves to almost breaking point... or give up.
I just want it for my little boy 😓😓😓

Really sorry to hear that, OP.

Personally I would fight it too - from what you have said (and I realise you've only posted a summary so you may have had these conversations), I still don't understand why they didn't wait for the eggs to reach the right size, and then use a lupron/buserelin trigger. Sorry I keep harping on about this but it's totally standard practice with PCOS and avoids OHSS.

Even if they don't give you a refund, but having those answers may help you decide whether to do another cycle with them or go elsewhere (if they don't have a good answer, I'd be concerned that they are not competent at treating people with PCOS and it would give you a chance to find somewhere that is).

PCOS should actually be a good thing with IVF and is associated with better outcomes... (When I went to my initial consultation my consultant said he'd prefer a polycystic ovary over a normal ovary if he had a choice)...

I’m so sorry this happened to you. Poor fertilisation is devastating. If the consultant lied then you should raise this again with the clinic. If you think they’re being lax around overstimulation, which can be fatal, look at contacting the HFEA maybe? 020 7291 8200 or email [email protected] They may not be able to deal with it but it’s worth getting that clinic on their radar. I would obviously also move clinics as trust is so important in this process.

Thanks all

I had initially accepted their decision and told them too but i did voice that i didnt agree with it.
But then, the next day, i read over the letter and i thought it just isnt right. So i called the lady that has been dealing with it.
I told her that i dont know what the common dosage is so did i pluck up that number (225iu) and get lucky.. of course not! I also said that nobody explained to me about the risks of doing egg collection early unlike what the letter states.
So, she is now going to bounce that back to my consultant but i wont find anything out until the new year. Again, i wont hold my breath.
We have decided to go again... it doesnt sit easy with me that we are going more into debt to do this, but i cant leave it on this note.. in limbo. So 1 more go.
Although, clearly PCOS isnt my clinics priority and they clearly struggle on how to deal with it, considering i got moderate OHSS the 1st time round, i have no choice but to go back there. Im ok with that as i do like the clinic. Its the cheapest in practically the whole of the UK with high success rates. Its local and i know them. Plus we have OH sperm there and to be quite frank, i cant be arsed shipping that to another clinic. The clinic were over cautious in me getting OHSS. And because this has happened, they will be very cautious with my cycle next time that this last cycle, doesnt repeat itself. I already have a plan in place, a lowered dosage (which for me, was admittence in itself that they did wrong). So, im optimistic that the next cycle, will be a more positive one.

Unfortunately, its not to a good start. My consultant replied to my email when i said i wanted to go again next month like he suggested, and since then, i havent heard from him (since weds) so i may miss the boat on starting in January. Which is ok, means i have another month of paying off the CC!!!!

Hi

Just an update...

I had a call from the clinic today regarding my appeal if their decision not to give me my money back or a discount on my next cycle.
They said they stand by their decision and were right to do collection when they did, however, as a goodwill gesture that have offered me a free scratch and free embryoscope, totalling almost £600..
Im satisfied with that.

Fingers crossed my next cycle, starting imminently, has more positive results.

Very best of luck OP.
I also felt my clinic was a bit blase about communication and kept changing their decisions about what to go for seemingly arbitrarily. It doesn't give you the greatest hope.

(We got lucky on 2nd cycle!)

Ha, probably the same clinic!

This is the same clinic where we got our DS so i still have a little bit of faith left. Luckily, both them and i will learn from the last cycle so pretty high chances, it wont happen again!
