How is it possible to inject an epidural drug into a woman's arm during chuldbirth?

How can anyone make such a mistake? I can't gety my head round how it happened!

Epidural infusion bags can be connected to ordinary drips.

There are now safeguards in place that were not there in the past. Bags locked in cupboard. Drip sets different colour. Different pumps to deliver infusion.

I can see how it happened in the past.

My ds2 has just gained a PhD in the field of patient safety. He's been published and is regularly asked to speak all over the world about this issue and is also carrying out more research so hopefully there will be fewer of such sad cases in future.

I have recently been in to hospital to have my 2nd child. The staffing levels were so woefully inadequate and they had way too much to do. I think this leads to mistakes.
