Is it possible to replace UPVC window pane seals?

Hi.
Our windows are pretty awful but we can't afford to replace them at the moment.
I thought I recently heard that you can remove the seal strips (4 of them) that are stuck around each pane and get new ones but I can't seem to find anything online that looks like what we need or how to carefully replace them?

Anyone done this or is it a job that needs a professional?

I don't think bleach will change them either - they have black mould in the corners and are discoloured cream, rather than the white that the window frames are.
Will try and find out more.
