I would make the patio doors one focal point, as that is what you will see when you enter the room.
You then need a main focal point when you are in the room. The problem with TVs is that they are pretty ugly and you don't want them to be the focal point of the room when you are sitting down socialising but, you want do want them to be the focal point when you are watching it
. If you can find a way to conceal the TV eg sliding panel, have it as part of a gallery wall, in a cupboard, or in a wall of shelves with multiple focal points of interest, that will help. I know someone who had a painting on a deep framed, stretched canvas that fitted over the flat screen TV. I don't think they were big TV viewers though.
I would have the TV on the left so it doesn't block the view/path to patio doors. It will make the room larger and airier, and be the least obtrusive place for the TV when you enter the room. Then a large sofa on the right so you have the maximum number of seats in the prime viewing position and armchair on the wall next to the door.
I wouldn't have a fireplace as it will mean that you have to have the TV hung too high for the best view, if you put it above it (and it will make the fireplace look ugly) or tucked off to the side, which will detract from the fireplace anyway.
Actually, I say "I would", I did. I have a similar layout and have tried multiple combinations. My TV is in a false wall to hide it.
