Really depends whether you are going to a civilised country where you can buy milk, nappies, clothes, bottles, sterilizing equipment etc or the middle of nowhere.

Their vaccination record
Good travel insurance
Pram/sling

We are going to Menorca, he is formula fed and temp will be around 24/25 degrees

Order formula to click & collect at the airport
Check security rules about baby milk and pack extra in case your flight is delayed, ditto nappies
Complete change of clothes for yourself and baby in hand luggage in case of mid-flight vomiting incident
A means of sterilising bottles and dummies
Swim nappies if they'll be going in the pool
Swimwear with built in sunprotection
Sunhat to wear with swimwear
Sunshade for buggy - I'd get one of those full cover ones rather than just a parasol type
Lightweight clothes and vest / sleepsuits - check if your accommodation has aircon to determine what you pack for nightwear.

I took both my DC to Southern Spain at a similar age, and cannot stress enough DON'T worry about lots of 'outfits'!! Just take plenty of funky vests/bodysuit things, and maybe light, soft cotton leggings and long sleeved tops for layering.

One of those blackout sunshades came in very handy for pushchair naps

Swim nappies, UV swimwear, sun hats

I'd say to bear in mind that Menorcans have babies too, so anything you forget can be replaced without panic. We're going away in November and I'm only planning to take a couple of days worth of nappies (more than we need in case of delays) and then buy a pack there. Ours is breastfed so I don't have to worry about food but formula is the one thing I'd take enough of in case you can't get the same brand (or find out what the local Mercadona in Menorca sell, Tripadvisor forums might help with this).

Get a GHIC for emergency public healthcare. You need one for every member of the family.

Identify where the nearest public healthcare centre is on Google Maps — look for UBS (small one), CS/Centre de Salud (medium), or PAC (24/7). You don't want to be doing this in real time if something is going wrong. The public health places don't advertise and may have almost no signs up to say what they are. If you ask your hotel for medical help they will probably call a private clinic/doctor/ambulance since they get commission. Of course, your travel insurance may cover that.

Pampers are called Dodot in Spain for some reason.

Baby formula is mostly different brands/formulations from the UK, even from the same multinational manufacturer. What brand do you normally buy?

If your hotel doesn't provide mosquito plug-ins for night time, get one from a Spar or other "superette" for about €6.
