Cape Cod definitely worth a visit. We did Boston with kids so not quite the same but we did a lot of walking (there’s a walking tour that’s good).

A day trip to Salem, home of the witch trials, is worth doing.

I was there last October and the weather was perfect for walking around. I was there for work so didn't see much, but I can tell you it was a big old waste of time taking my warm coat!

October is perfect, make sure you walk across the bridge (or get the underground) to Cambridge and go and see Harvard Square. If you rent a car you can get out to some of the forests which will look amazing that time of year. Boston itself has some lovely museums and good shopping. My parents lived there for a while and it's the only American city I'd visit again

We went mid October for a week last year. Weather was perfect really, cold but crisp dry and sunny every day. And the Fall colours were just stunning!

October is great, all the attractions are quiet, the weather is fine, not too hot, not too cold. Good attractions for kids - the Aquarium, science museum, the Constitution (oldest commissioned warship in the world), Bunker Hill monument and the tour of Fenway Park (if they like sports). Both the Duck Tour and the audio tour at the top of the prudential tower give a good orientation of the city. If your children will tolerate an art gallery the Isabella Stewart Gardiner is my favourite. If they’re very art gallery tolerant the Museum of Fine Art is spectacular. Boston is very walkable but the T (underground) is very easy to use. You would be able to take in a basketball game at TD Gardens, which is a great experience.

October is peak fall season so you'll see the amazing colours (head up into the mountains for a day if the trees are still green in the city). It's a great time to visit. I love Sept and Oct in the NE US.

I went last year over Halloween and there was a lot of rain but it was still wonderful to see the famous fall colours. Loved Salem!
Also it’s worth getting an Uber to the Samuel Adams brewery tour if you like beer - it’s free and you get free beer!

As pp says the Fenway Park tour is great. Would be even better if you can catch the end of the baseball season and go to a game. Lots of nice bars/restaurants in Newbury Street.

I misread that you were coming without children. For just adults I might recommend cocktails in the restaurant on the 50th floor of the prudential tower rather than the observation deck. Also the MFA is amazing and has a good restaurant for a nice lunch (as has the Isabella Stewart Gardiner). I’m not very fond of Salem, but the Chinese house in the Peabody museum is fascinating, easiest way to get to Salem is by train from north station . You could consider getting the train to Portland, Maine for a couple of days; it’s a lovely little city with great bars and restaurants and lots of small independent shops. We did a foodie walking tour which was very good.
