Doctors certficates - whats the deal?

I've got glandular fever. I have been ill for six weeks so far (and battled through work and study and looking after the kids over school holidays without a single day off).

I have finished up at my job now (contract came to an end) and have a month left of college until end of term. Last week I got really ill (very high temp, delirious etc) and took a few days off college. My tutors are really sympathetic and have said I should do reduced college hours until I feel better. They have also said I can defer handing in my last assignment and doing last exam until September if I get a doctors cert. Thought long and hard about it and have decided it would prob be best to defer the exam at the very least. So went to doc's to get a cert and she was really arsy about it. Made me feel like I am trying to pull a fast one. It's not even like I am getting paid days off work - I have paid £3,000 for my course! Plus I have a temp and pus dribblig down the back of my throat. I am ILL!

In the end she reluctantly scribbled a note saying I had a viral illness (nothing else).

Is this what doctor's do these days? What are they obliged (or not?) to do. I don't really understand the whole medical cert thing.
