Hi

I just don't know what to do

Someone told me to go to a&e because of potential kidney issues???. and I think they're being unreasonable so seeking other opinions

5 days ago I came out of resus/hospital after a life threatening asthma attack. I had a lactate level of 8.7. potassium 2.1 and oxygen on ABG of 8. My BP at one point was 65/57... basically really really unwell.

The issue is, now I am in so so much pain. I feel more systemically unwell than in hospital... But breathing fine.

Physically everywhere hurts especially my calves. I haven't felt pain like it. It's like flu on steroids but I've not got flu. Today I found it difficult to walk my knees hurt so much. It's dreadful.

I did a urine test and I am showing++ protein and mild blood but no infection markers. I also have no pelvic pain etc. fluke?

I feel absolutely shocking. Ive had many asthma attacks but never ever felt this terrible. It hurts to move.

111? Ignore? A&E? Attempt GP appointment next week? I must say I would feel BEYOND ridiculous going to a&e.

If you feel systemically unwell with no cause then you need to be seen. 111

Actually this happened to me and it was chronically low potassium. It can be deadly. You need to be seen tonight. I could barely walk.

111 they might get you an out of hours GP appt. I’ve had that a few times when I didn’t want AE and couldn’t see a gp

yup, deffo get medical advice today. Plenty of things it could be, depending on how long it took to turn the asthma attack around (and how long you're BP was like that) and what drugs they gave you. It would be unusual to be discharged from resus after a life-threatening asthma attack so guessing there's a bit more to the story. (anaesth/icu doc fwiw)

Can you bare to go back to A&E? I'd be concerned about the leg pains and a GP /111would send you there for bloods anyway if they were worried tonight.

Don't worry about the current industrial action - the A&E I work in is fully staffed with consultants in emergency medicine and they're being pretty efficient at the moment (plus people are generally staying away...)

I'm a paramedic if i went to you with your recent history and your presenting symptoms now id send you straight back in to A&E, I'm pretty sure GP OOH would do the same (i certainly wouldn't bother waiting an hour for a call back to discuss you with them just to be told to take you through anyway).

Back to hospital OP, sorry.

I'm a nurse and I triage 999 calls. Obviously I don't have full information, but from what you've said I'd be telling you to get yourself to A&E.

Hi all

I am quite surprised by the response. I thought to be told to wait it out!

Sorry for any confusion :
I was discharged with an appointment with my respiratory consultant coming up and when all my blood/breathing returned to normal

So they aren't still 2.1 etc etc they were normal on discharge

I still feel bad and still showing blood/protein in urine. I don't know if I can go back and just say 'hi I feel terrible ' that's not life threatening is it?! I don't have a fever. I just feel terrible pain. And exhaustion.

Plus it's the weekend. Eugh.

Out of hours doctor and tell them you think you have a uti/kidney infection due to your symptoms and get antibiotics off them!!

Honestly medical professionals x3 have told you 111 minimum, preferably A&E. Just do as you’re told.

That much pain with no known cause should be checked out OP. You should definitely go back to A&E. A couple of years ago I had horrible pain with no known cause, turned out I was badly ill with sepsis. A blood clot was also initially suspected so you could have either of those or something else that urgently needs seeing too.

A UTI that hits your kidneys can make you deeply unwell for a prolonged period and much harder to treat.

Please seek some medical advice, the longer you leave this the more unwell you could become - plus the closer to Christmas.

Because if you are feeling unwell this soon after those blood results no HCP is going to discharge you without repeat bloods and the only place your going to get urgent/ out of hours bloods is A&E.

They sent me to the OOH GP who gave me antibiotics for UTI, unfortunately no bloods or anything and wasn't even slightly interested about my hospital admission and how maybe this body pain is related. I think he was very tried. At least that's the impression I got.

I will take them and reassess on Monday... Unless things drastically change

You mention you have asthma - have you had a lot of steroids for this (over time, not necessarily just now)? If so this may be adrenal insufficiency, have you been tested for this? You can suggest it to whoever is treating you and ask them to investigate. Ideally you need an early morning 8-9am cortisol blood test (this can be arranged by your GP). Have a google of secondary adrenal insufficiency.
