After everything, could this be IBS after all?

I’m really at my wits’ end and would be so grateful for any advice or shared experiences.

To give a bit of background: I’ve been struggling with severe pelvic pain along with bladder and bowel problems for several years. Initially, doctors believed the cause was a large fibroid and adenomyosis. I had a hysterectomy 18 months ago, which initially made a big difference — my symptoms improved massively. However, about eight weeks later I started HRT, and unfortunately, all of my symptoms came back with a vengeance.

My gynaecologist suspected microscopic endometriosis, as nothing was found during surgery or on scans. Since then, I’ve been seen by several specialists, including colorectal, but all of my tests have come back normal.

My current symptoms are:

Constant, severe lower pelvic pain that radiates to my coccyx and the fronts of my thighs.

Bowel pain that feels like passing glass, with much worse pelvic pain afterwards.

Constipation unless I take Laxido — but sometimes I swing to diarrhoea.

Rectal bleeding (investigated but no clear cause found).

Bowel urgency, and recently I even had an accident, which was very distressing.

Bladder irritation that feels like a constant UTI, though tests are negative.

Gynaecology have now discharged me following a recent MRI, but I’m still in daily pain. I had to leave my job a year ago because it’s become unbearable. I’m currently on Co-dydramol, Gabapentin, and Naproxen — the Gabapentin has helped a lot, but I’m still struggling.

Emotionally, I feel completely drained and can’t see a way forward. I avoid eating when I’m out because of the pain and urgency, and my quality of life feels so limited. I’m now wondering whether IBS could be part of the problem.

I wonder if there might be different things going on. Bowel issues, particularly constipation, can cause uti. The nhs is terrible for failing to spot what is now termed an embedded uti. It needs more advanced tests to pick up and they don’t do them. Look up CUTi or embedded uti, Facebook has good groups for this. That might account for your bladder symptoms. Needs longer term antibiotics. Bowel is trickier. Yes, it could be ibs, also worth looking at SIBO or SIFO, other often overlooked diagnosies.

really hope you find some answers. I have suffered with both bowel and bladder issues and it’s miserable.

Seeing as your symptoms recurred after starting HRT have you tried going without it to see if there is any improvement? When I was on combined HRT patches I suffered terrible bowel pains which I had never had in my life before and within only days of removing the patches I didn't have pains. Just a thought.

How's your pelvic floor? If it's been tense for a long time it will be weak and this can cause accidents and what seems like sensitive bladder. The tension could be cause the pelvis pain.

I had a problem with mine a few years ago, tension was making a BM impossible. GP tried everything under the sun even a scope and in the end I figured out it was my tight pelvic floor and had to sort it out myself in the end with self massage, stretches and strengthening exercises. GPs seem not to know anything about pelvic floor dysfunction.

Thank you. This keeps coming up whilst researching. I am definitely going to see a pelvic physio and see if it helps. It does make sense x

My bladder is much better due to treating the embedded uti. The first 2 urologists I saw didn’t believe in it either. I just wasn’t happy with their ‘solution’ which was for me to suffer indefinitely. You might find the below info helpful.
