Can't get rid of baby's scabies - help!

Baby (10m.o) has been suffering from scabies for about 2.5 months now, since early July. We've been to 10+ doctors appointments now, used permethrin cream 5 times and crotamiton lotion once for 6 days straight and the rash is still as bad as ever. Each time after treatment we've waited at least 2 weeks to see if there's an improvement but it always comes back in full force. I've been able to clear it with ivermectin pills but I catch it back from her each time - she has never been cleared. I do all the usual things - hot washing sheets, towels, clothes etc. I've used hydrocortisone on her spots and they fade a little but come right back again. Does anyone have any experience with this and tips on how to solve it? She can't go to nursery because of the rash and it's a nightmare for my work. TIA.

How do you know it's scabies? Its really unusual for it to carry on this long after this much treatment. I'd be asking for a referral to a skin specialist tbh, or just paying to see one privately if that's an option.

Are you sure she doesn’t have eczema? Do you have any photos of the rash?

Yes, unfortunately. I had scabies first (confirmed by a dermatologist) and passed it to her due to my first treatment failing. No history of eczema in our family at all and her pediatrician also says it's consistent with scabies. I keep clearing scabies and getting it back too.

We are going to the pediatric dermatologist on Tuesday and I pray she has some real answers. I would think the ongoing rash was maybe no longer scabies (a reaction to the cream or irritation) but a) it keeps getting worse - it gets better for about 2 weeks after the treatment then worsens again and b) I keep getting it back off her - I get cleared and then get the characteristic burrows and itching again too. I am pretty desperate now as I pay for childcare and can't use it, nor go to the office.

You might need to ask for referral to paediatric infectious diseases to get ivermectin off label. Where in the country are you?

We're finally seeing a pediatric dermatologist on Tuesday, so more specialised than just the pediatrician/GPs. I am hoping she may be able to prescribe ivermectin off label or at least a different topical. And obviously to rule out any other cause of the rash, although I don't think it's something else.
