Nappy rash . . . when should I take dd to the doc?

DD - 9 months old and teething has had nappy rash for the past week. I am using Bepanthan cream every nappy change, using disposable nappies. But the rash is getting worse. Today the rashy bumps are starting to break open. I have switched from the normal Bepanthan to the Antiseptic one, so will see how she is in the morning but when would you take a baby to the docs? She doesn't seem to be in any great pain apart from getting distressed after having a poo. Perfectly reasonable of course, poor darling.

Grateful for any advice. Also anyone used any miracle creams they would like to recommend???

Take her to the doctor because he can give you a very good cream called Timodene, which is prescription only, and is an antiseptic, antifungal, antibiotic and mild steroid cream. The nappy rash might have turned into thrush. Other than that, Metanium is fantastic and is sold in Boots. Yellow tube.

If you take your dd to the health visitor they can have a look and prescribe some mcreams as long as 1 of them is able to do that.

Thanks so much for the advice and thank you DisplacmentActivity for the link, full of useful advice. I will be whisking up the egg whites!

I think I will be taking the little darling to the doc tomorrow if no better. Has been a few days now and is just getting worse.

I would recommend metanium 1st line and then go to timodene/trimovate on prescription if not settling.

I had this last Sunday. Ended up taking DD to A&E where they pronounced it thrush and gave me a prescription for canesten (that I'd asked the pharmacist for in the first place and he wouldn't sell me )

It sounds like a nasty case of thrush. I wouldn't rush out and buy another nappy cream as bepanthen is fine for normal nappy rash. But, it would definitely be worth trying some 1% canesten cream/own brand clotrimazole. Also, try not to use wipes but cotton wool and water. All wipes, even sensitive ones, contain a small amount of alcohol in them which is painful and irritating to sore/broken skin. Obviously, make sure that you gently and thoroughly dry the area and try to leave your DD's bottom to air for a short while without a nappy on. Even a couple of minutes helps. My DD had this a couple of weeks ago and the worst of it cleared up very quickly with just two applications of canesten a day and then using zinc and castor oil after washing her with all other nappy changes. If you don't already change your DD's nappy every 2-3 hours and as soon as she has filled her nappy then you need to do this whilst she has broken areas of skin.

I hope this helps. Oh, and while I think about it, if you do want to buy a nappy cream (eg metanium, sudacrem) rather than an ointment then please, please make sure that you spread the cream thinly and evenly. I work in a dermatology department and have seen the most horrendous case of infected thrush caused by a carer applying too much metanium which caked slightly in the patient's skin creases. This allows skin to get sweaty and traps the sweat which is perfect breeding conditions for thrush and bacterial infections. Most patients with bad 'nappy rash' in hospital respond well to zinc and castor oil, regularly pad changes and thorough cleaning.

there's a cream that boots sell behind the counter called morhulin - it's fab. if that doesn't shift it make an appointment at the docs.

Kavlon (made by 3M) make a barrier spray which is great to get over the soreness and give the skin a chance to heal. I personally wouldn't use this long term, but they also make it in a cream which works really well once the sores have cleared up.

I'm not sure this can be bought over the counter though.

Hi - if you are using wipes then maybe stop and use cotton wool and cooled boiled water - this really helped my baby - also lots and lots of naked play on the changing mat.....also another big fan of metanium after every change, i also chaned nappy every hour!!!! mumsnetter advice and it worked - good luck

Fresh air did the trick in my house. Used those pampers changing mats and let dd lie on it for around an hour a time a couple of times a day with her bum exposed to the elements. It worked wonders

aaaaah mumsnet girls you are all so helpful! Thank you.

My poor little darlings bum is still fairly sore, so following mumsnet advice I will be using cotton wool and fresh air as well as the cream.

I took her to the pharmacy yesterday and they advised that I use the antiseptic Bepanthan PLUS Sudacreme for 24 hrs then take her in there to show off her bum to the pharmacist if no better.

Do you think that nappy rash is related to teething, I have been blaming it on this as dd has been a bit crotchety and dribbly for weeks now but no teeth seem to be coming through. Maybe she is just in a bit of a mood these days .

I have had babies with nappy rash.You should try to leave there nappy off and let lots of fresh air to her botty.Rashes are related to teething.Also a old fashion remedy for nappy rash is egg white.It seals the area so it does not get more sore.Nothing more worse than a baby teething and in pain.Poor baby.Also lots of fluid (water etc.)It will dilute the urine so it will not sting when they wee. xx

sudocreme always made ds' nappy rash worse. i would use the antispetic bepanthen on it's own.

Had to take my precious to hospital when she was 3 weeks old her nappy rash was that bad (bleeding etc). Believe it started in the hospital as used certain wipes instead of cotton wool and water (as instructd by midwife.)Never use wipes if there is nappy rash.

We tried Trimovate and something else before eventually got prescribed Canesten which cleared it up in about 3 days. Also huge fan of thinly spread metanium followed by Zinc and Castor Oil. Have been told by doc to avoid Sudocreme as full of irritants. My little treasure still very prone but usually is now treateable with combination of Metanium and Zinc and Castor Oil. Also make sure lots of nappy free time. We always use Zinc and Castor Oil as her barrier cream now.

Bathing the bum in water with some bicarbonate of soda in it works for us!
