Health Folder or Portable Action Plan ?

Hi during a recent emergency I was asked by 111 if I had a folder for my adult daughter for use in emergencies. She has multiple complex Chronic and rare medical conditions which also inc learning and processing issues .
I have no idea how to obtain one !
Does anybody know ?

For my mum I have her repeat prescription list and a print out of her medical record summary which always seems to be helpful.

Edited to add that I was able to print the summary from her electronic record on Emis, but in the past I've got it from the surgery- just had to pay so much per page (Can't remember how much but like a photocopying charge)

You can fill them out yourself- there are templates for them online.

It is very helpful for staff looking after people with learning disabilities and complex medical history to be able to see everything written down, and you can fill in exactly what they do and don't usually need help with.

I’ve no idea if such a thing exists. But my take on it is that are asking if there’s a handy, easily available source of information on your daughter’s conditions.
What I have done (in the past) for three adult family members- which the emergency services said they loved was:

  1. obtained a large colourful A4 file with dividers. Inside in section I placed letters from hospitals/drs regarding diagnosis, treatment plans and ongoing appointments. In date order and divided up by diagnosis.
    i had a section for my personal notes and that of carers - like a diary noting any changes and possible explanations. A bit like hand over notes between carers? I also had a section for daily obs such as blood pressure, temperature etc . You may or may not need to do those or maybe only if unwell with an infection? Finally for those with a longer term illness, I added a section ‘this is me’. I also used the Herbert protocol here and added information such as favourite foods, to shows, clothing they often wore, where they may have worked, hobbies, some pictures of them etc.
  2. a large bold note on the front of the fridge stating where this folder was kept. And it was always kept in the same place. Unless accompanying someone to an appointment.
  3. a digital copy of the same file I kept on my own laptop - basically took a photo every week/new letter appeared. Just in case the main file was ever lost. Yes my digital file may have been a week or so behind, but it meant a new file could be physically recreated within an hour or so.

Don’t know if this helps and yes, a little time consuming, but it’s what I did and seemed to work very well.

Afaik they're not centrally registered or anything but helpful to have critical info handy, especially if the patient isn't able to express what's wrong/different to their normal.

The person who asked you about it should have informed you how to get one though imo!

Hospital passport is what you need by the sound of it. Lots of Trusts/charities have downloadable templates with prompts/ideas you can fill in.
