Should we complain about how GP is handling gout treatment ?

My DH has gout, and he is currently on allopurinol 400 mg. His latest monthly serum urate blood test was 530 μmol/l. Which is still high; however, the GP has marked it as "normal, no further action required." But I feel he should have the dose increased to 500 mg as per the guidelines below.

According to NICE guidlines If >360μmol/l increase to 300 mg/day
o Continue monthly sUA levels, increasing by a further 100mg/day each month until sUA <360
μmol/l or maximum tolerated dose is reached (The BNF max dose = 900 mg per day,8 but
locally it has been agreed that treatment should be reviewed at 600 mg per day)
o Once sUA target is achieved, annual sUA measurements are recommended

I wouldn’t complain. But I’d book another appointment and explain that I wanted to continue increasing the allopurinol until the level was under 360, and ask what their thoughts were (maybe there’s a reason they think it’s okay to keep things as they are).

How is he symptomatically?

Surely having a simple conversation with the GP about the issue would allow them to address it? Why would you need to submit a complaint?
