Raised liver enzymes

Would anybody please be able to reassure me about my blood test results?

I am 35 years old, 5ft 8inches, 192lbs, non smoker, non drinker, low activity levels if I’m honest (around 7000 steps per day so could do better!)

Originally I went to my GP in April with heart palpitations and feeling very lethargic and down/anxious. She explained it could potentially be a variety of things including issues with my thyroid, low iron etc. So she booked me in for my first set of bloods.

My blood test came back with the following as being raised:
ALP: 179
ALT: 65
My iron and folate were also low.

From this I started iron and folate supplements and the GP booked me in for an upper abdominal ultrasound to check for NAFLD. All was fine - no issues with my liver or surrounding organs on the ultrasound.

I’ve now had a follow up blood test specifically for the liver and it’s now thrown up the following results:
ALP: 132 (highest normal threshold being 130)
ALT: 33
But my GGT (which I’ve obviously never seen before!) is 64 u/L and the normal threshold is 40! I’m now freaking out again as I have huge health anxiety. I now have to have another blood test in 4 weeks and I’m just really worried about what is potentially wrong with me. I had my son 2 years ago and all I can think about is that there’s something seriously wrong (I know that’s not rational!) thank you for any of your help in advance.

Have you felt any better on the lethargy since taking iron and folate? Does it feel as though that is working?

I would consider that ALP has come down really well and AST is about right now too. Hopefully this will be consistent on the next test or even lower. Even just being anaemic can disrupt everything and take a while to get back to normal, so that could be fixed just with the iron and folate.

Is it a fasting blood test, or are you allowed to eat first? If they’ve not requested fasting, I always still get blood test as early as possible in the morning to give my body the chance to put things straight overnight first. You’ve said you’re a non-drinker so it’s not a case of staying off the wine the night before!

GGT is a more unusual one, but if the doctor was really worried about it he’d have suggested something immediate not just another test in four weeks.

The trouble with four week gaps between tests means that if it’s being affected by hormones then you’ll be back in about the same point in your cycle, but they always do that without thinking about it.

The ultrasound being clear is very good news.

It sounds like they’re doing a thorough investigation, so if there is anything wrong then they’ll find it. My biggest concern is when they send people away without listening.

Thank you for your reply! Yeah I have felt a bit less lethargic since the iron and folate supplements actually but not 100% yet! Hopefully it's just that being off that's created all of these liver issues...

I'm trying to be positive and rationalise things as I guess the ALT and ALP going down to pretty much normal is a good sign.

My weight is probably a huge factor in this tbh!

Thank you for your advice ☺️

HI, sorry to join in here. Came across this post about raised liver level. My blood tests also shows raised ALT too . How did it come down and what did your doctor say about the raised liver level?Do you have to eat or avoid certain food?
Thanks.
