Scalp rash help - ringworm or psoriasis?

Posting on behalf of Dh has a rash covering a large area of his scalp. I can’t tell whether it’s seborrheic dermatitis, ringworm or psoriasis - from searching images of each comes up with results that look similar to this. It’s been on and off for over a few years now. He’s been to gp, they initially gave anti fungal cream and nizoral shampoo but they caused inflammation on his skin. He’s used T gel since which doesn’t irritate his skin but hasn’t eradicated it either. Also has smaller patches on his chest, behind ears and in beard on sides of face, but not in groin area. He thinks it seems to get worse when he’s stressed, but it could be the other way round the rash is stressing him out. I’m telling him he needs to go back to gp maybe ask for oral anti fungal and for a referral to a dermatologist to find out exactly which it is, but he needs to treat it now as I’m sure this could take a while, and he’s self conscious about it. Any suggestions? Is hydrocortisone worth a try?

I have psoriasis and his scalp doesn’t look particularly like psoriasis to me although your description of locations and when it gets worse do. I use T-Gel and it helps but never clears it, my scalp hasn’t been clear in decades despite constant T-Gel use. Best have a trip to the GP.

Cocois may well help if it is psoriasis. I think the wait to see a dermatologist would be a long one unless you can go private.

Thanks @Coughsweet , will show him the cocois. Thinking seborrheic dermatitis could be most likely, hoping it not ringworm as it’s so difficult to shift and contagious.

Doesn’t look like my psoriasis although I can’t really see it when it’s on my scalp! Mine are raised scaly scabs on my arms and legs and the ones on my scalp feel scaly as well. They flake so I sometimes have large white bits in my hair, like dandruff but bigger!

The only creams I use that keeps them at bay are steroid based, if I get a new lesion and can get cream on it straight away, it does clear up in a few weeks. For my scalp, I have a liquid steroid like Beetnovate and put it on late in the evening and wash my hair in the morning.

I have psoriasis mainly on my scalp, I keep it under control with Alphosyl shampoo.
