What could cause episodes of sudden extreme tiredness and finger tingling?

DH (early 30s) as had various health issues since I've known him. Chronic migraines, digestive issues, allergies. No diagnosis has ever been found and the GPs don't seem to take him seriously. A lot of his issues flare up every so often so he does have periods better than others.

Last year we all had some sort of flu/Covid thing and he never seemed to properly recover. He was very weak and tired so I guessed long Covid. He'd get chest pain and extremely breathless on exertion. It was six months after we were ill and he was often barely able to walk across the room. GPs looked into his heart due to family history. A couple of things showed on the echocardiogram but they were marked trivial and GP doesn't believe they'd cause his symptoms. In fact I fear GP doesn't believe his symptoms full stop.

He takes antidepressants and has tried therapy several times. I do not believe it is caused by anxiety primarily because it seems the opposite. Generally he will be doing really well and be happy and then it'll hit him again and over time it gets him down.

The last few days after things seemed to be improving they've got worse again. He's started suddenly getting extreme fatigue out of nowhere especially in the late afternoon/early evening. He will be fine all day then it will just hit him out of nowhere. When it happens he says he feels like he's going to pass out and he looks really unwell and not right. He starts feeling "tingling" in his fingers. His oxygen saturation and blood pressure are fine when this happens. I'm used to his many symptoms but seeing this is quite concerning.

It's now happened for two afternoons in a row. I kind of want him to try the GP again and he's agreed but is worried that it will further solidify the idea it's all in his head if they can't find what it is.

I'm obviously not asking for anyone to diagnose him but wondering if anyone's able to guide with what types of things it might be and what tests to push for if needed.

Thank you. I've never heard of this but had all of the symptoms of this. This is definitely one option to follow up on. I actually don't think they've ever checked for deficiencies like B12 unless that would shown up in a general full blood count.

Tingling’s another of those infuriatingly vague symptoms GPS don’t like.

Could be anything from stress to MS. He’ll have to have it more than twice to see a GP tho - mostly it’s nothing serious and it passes.

Has he had his vitamin B12 levels checked? DD had very similar symptoms - ended up being pernicious anaemia. She has to have a B12 injection every 3 months for life now

Has he had a thyroid Function test? If not, request one. Those symptoms, including the tingling, can be a sign of underactive thyroid.

Sounds like B12 is a good one to look into.

MS comes up a lot when googling but honestly I have no idea. The symptoms feel so vague and everyone's experiences of it sound so totally different that it's hard to tell but while googling I've not come across anything that's made me think it sounds just like what he has.

He was diagnosed with an under active thyroid as a teen but was confused because he's always been very slim so assumed it would be overactive if anything. He stopped taking his medication as he felt "it didn't help" and when as an adult he has had his thyroid checked they've said it's fine. I don't know if thyroid is something that gets better in it's own or not but the blood test he had recently definitely looked for something thyroidy and came back normal.

Some what you describe is my DH - he has recently been diagnosed as having coeliac disease.

He went to the Drs about something completely unrelated. They did various blood tests and those showed he has coeliac disease.

Having done a lot of reading up on it his various (relatively minor) ailments now all make sense!

Sounds like my symptoms before B12 deficiency was diagnosed, I was also screened for coeliac as I have digestive issues. Blood tests from GP are needed, best of luck.
