Does anyone know what might be wrong with me?

Hi all, I'm wondering if anyone has had these or similar symptoms before and may be able to help me work out what might be wrong.

I have had some gynae issues recently - irregular periods, heavy bleeding, pain, intermittent bleeding - and saw my GP about these. They referred me for an ultrasound and gave me some painkillers and advised me to go back if anything changed.

Last week I woke up feeling fine, had a shower all normal. Went to the loo and felt slightly constipated, not bad but in the sense that I wanted to go but couldn't if that makes sense? I didn't force or strain.
All of a sudden I got a wave of severe pain in my lower abdomen. I managed to open my bowels, not a lot but as I felt I needed to.
With the pain, I was sweaty, clammy and lightheaded. It was in waves across my lower stomach and I felt nauseous with it. I could barely move, had to lie on the floor with it. Standing up felt like pressure on my stomach and made the pain worse.
I managed to get up and dressed after a couple of hours, and went back to my GP. As soon as she touched my stomach to examine me I was in agony, I couldn't bare to be touched. She sent me to a&e.

In a&e, the pain was no better and the doctors wanted to rule out appendix etc. I had 10mg of IV morphine which barely touched the pain.
They did a CT scan which showed nothing of any concern, but the pain wasn't going.
Due to the recent gynae history they referred me to gynaecology and also did an ultrasound. The ultrasound showed a displaced coil, a very thick uterine lining and some free fluid in my womb. They also said I had a 3.5cm blood filled cyst on my left ovary, and 5cm parenchymal (?sp - the type with hair and teeth etc) on my right ovary.
They said nothing obvious should have caused the severe pain, and the pain was higher than would be expected for gynae issues anyway.

The pain is mostly under my belly button, lower middle abdomen but also about 3 inches to either side. Not around my sides or back at all.

It's now almost a week later. The pain isn't as severe, I'm managing with paracetamol, ibuprofen and codeine but I can definitely feel when they are starting to wear off. It's like a constant ache, with waves of worse pain. I also feel like a spasm when I walk when my feet hit the floor, like it is "jarring" me.

Does anyone have any ideas or had anything similar as the doctors seem stumped?

Isn't it the cyst? They can hurt in the entire pelvic region. Are they not concerned about it?

Cysts can be very painful.

You may have had another one which burst and hence wasn't seen in the ultrasound. From personal experience that can be hella painful,

Oh and I forgot to say, other issues that may / may not be relevant:

Stomach pain when I urinate. Not like a stinging/ burning, like a dull ache in my actual stomach.
Reduced appetite and feeling full very quickly.
Bruising very easily.
Keep getting breakouts of boils/large spots on my bum which are extremely painful.

My blood test showed an infection so I'm on antibiotics, nothing else of note but I don't actually know what they tested.
GP did swabs for Chlamydia and gonorrhoea which were both negative.
Not pregnant, confirmed by 2 urine tests and a blood test.

The surgical team discharged me before gynae could see me.
They phoned me and said basically scan in 3 months and if it's bigger they'll see me again, if not they won't.

They seemed to think the cysts were an incidental finding as the pain is more in my stomach rather than pelvis, but they couldn't say for sure.

Yeah, no issues there. Other than the slight feeling of constipation on the day the pain started my bowels have been normal.

I'm taking regular movicol now because of the codeine too.

I think you should go back to the doctor op. Write down all the symptoms you're experiencing.

So many things there!

Displaced coil - where is it sitting? Are they going to remove it?
Cysts - any plan in place for these?
Infection - where is the infection?

Any possibility of it being kidney stones?

  • Coil is "too low" and they said wouldn't prevent pregnancy. They aren't sure if the displacement will be causing the pain. They didn't do anything about it but I've contacted the clinic where it was fitted and they have arranged for me to go tomorrow for removal.
  • cyst is just a wait and see by the sounds of it. They gave me the option of asking my GP to arrange a repeat ultrasound in 3 months and requesting a gynae referral if they have grown, or they would arrange an ultrasound and follow up directly. I chose that option to guarantee the follow up.
  • infection they haven't said where. Just that my bloods show an infection, they've given me co-amoxiclav which should basically cover for any source of infection. The blood tests they did after the first 24 hours of IV antibiotic showed that my blood levels were getting better. WCC 19.7, CRP 3 on admission, after antibiotics WCC 9.8, CRP 97 but they said the CRP has a "lag" time so that was too be expected. They didn't recheck any bloods after that.

I spoke to my GP this morning to get a sick note for work and they have advised me to try and wean off the painkillers and go back if needed. I'm going to reduce the codeine to 2 morning and 2 night, with 1 at lunch and 1 at tea to see how I manage. My husband wants me to go back Friday if the pain isn't easing because that will be 10 days and the antibiotics are due to end on Thursday morning. I've agreed with him that it sounds like a good plan because I should be feeling better by then.

Oh and kidneys looked fine on CT scan and basic abdominal ultrasound, as did liver, gallbladder, appendix and spleen.

They seem pretty stumped by it because there's nothing "obvious" wrong in the area where the pain is.

Had an appointment this morning for my coil removal. They couldn't find it.
Lota of pain when they put pressure on my pelvic area, and absolute agony when they were searching for the coil.

Yesterday I had a bout of very heavy bleeding, I bled through a super plus tampon and 2 sanitary towels in under 1.5 hours. Very watery, no clots. No bleeding at all since.
Strings were there yesterday afternoon after the bleeding because I checked. I've not had anything fall out that I've noticed either. I did have a very painful spasm type pain this morning that felt like a knife across my stomach but nothing like that since.
I'm in pain from them looking for the coil, but the general pain seems to have eased a bit, it isn't as obvious when the painkillers are due to wear off. I'm also managing with 1 codeine during the day instead of 2.

They want me to have an urgent ultrasound to see if the coil is still there but not sure what else they can do in the meantime.
