Very itchy bottom

A bit embarrassing, but I have a very itchy bum! Between the cheeks and around the anus. I have IBD, so I am used to bottom-related issues, and this pales in comparison to what I’ve had to put up with in the past with that disease. However it’s been going on for over 2 months now so I am beginning to wonder if it’s more than just irritation.

Once I have a shower in the morning it’s pretty much fine through the day, but it slowly gets worse into the evening. It’s been waking me up during the night and it was so bad yesterday I couldn’t get to sleep. I only wipe once a day, if that; I try to just use the shower in the morning instead if possible. And it’s normal for people to wipe their bottoms without experiencing this effect!

Previous advice on here points to worms. But no one else in the house has reported any symptoms and I’ve never seen any (and with IBD, I am very much used to examining my poo in great detail). I carried out an extensive analysis of my poo this morning. But I couldn’t see anything immediately obvious. I think it was just sesame seeds (sorry TMI) and fibrous bits. But then again I’ve read that they may not even be visible.

Advice please!

I'd take some threadworm medication to see if it helps. Worms aren't always visible.

If it does help then treat the whole family.

Eczema, try sudocrem/ prescribed steroids. It's autoimmune related so fits with previous IBS

Treat yourself and everyone in your household for worms anyway with Ovex and strip all the beds, wash all towels, clean and dust etc.

My daughter gets a really itchy bum when she has them but I've only ever seen one in her nappies etc! They can be sneaky buggers. The fact that your symptoms get worse in the evening are a big red flag.

Piles can sometimes feel itchy (inflammation).

And/or have you trued cutting things out of your diet? Sugar, dairy… one thing at a time to see if that alleviates.

Could be psoriasis OP. It drives me crazy with the itchy in that area. I have to use strong steroid cream intermittently (because it thins the skin) but it keeps coming back.

No harm in trying the worm tablets honestly!
just take them. You often can’t see the worms.
I’ve had it quite a few times and thought it was thrush or piles flaring up but every time the worm tablets have sorted me out.

I had this recently - tried worm tablets, no help! Went to the docs- piles! Internal ones I couldn’t see/feel (other than the god awful itching). Double course of 7 day suppositories and I haven’t looked back!

Could it possibly be too vigorous washing in the shower leading to very dry skin? I've had this and applying a blob of gentle rich moisturiser to the anus every morning has helped massively.
