Trauma - forearm cut and operated on- who is the specialist

Hi! I’m asking for my sister. Her family lives in the Middle East and her husband was recently involved in an accident at work (he runs his own business) essentially his artery on his right arm was cut as well as his tendons ‘flexor digitorus’. ‘Ulnar artery’ where operated on and there was a lot of blood loss. He’s in a lot of pain however the hospital he went to is refusing to treat him due to their health insurance status. He has travelling back to his home country for treatment but is at a loss as he doesn’t know what specialist to see? Orthopaedic surgeon? He just wants them to have a look at the area and confirm surgery was a success and ensure all is as it should be. His home countries healthcare system doesn’t work with referrals you basically have to figure out the specialist on your own. So any advice on what specialist he should see? He is in quite a lot of pain so they want to figure it out urgently.

Either orthopaedics or plastic surgery. It varies slightly in different places - in the UK I'd say to go to any hospital which is a Major Trauma Centre, and they'd decide it themselves, but I'm guessing that's not the system where he's going?

Plastics-ortho don't usually deal with nerves

No, no major trauma centres unfortunately. I think he will start with a general physician and take it from there. He has already been operated on and is in a plaster cast…you just want the follow up care and reassurance from the right place .

ideally plastics/maybe ortho/vascular if no one else is interested! Not a general physician.
Hope he's got a copy of a discharge summary/op note and maybe photos of the injury. You'll probably be able to work out which specialty patched him up in the first place from the paper work and take it from there?

He has the discharge papers but it’s all very vague. No mention of follow up required on the discharge papers only to stay in a cast for 4 weeks and the ‘department’ only mentioned ‘emergency trauma surgery’ . It was a military hospital and they only took him on as an emergency case as he probably would not have made it to another hospital due to the lose of blood. They said they could not do anything for him after discharge.

the ortho department have said it wouldn’t really be them (this was my first thought too) and vascular - it seems more than that no? 😓 he’s only in his early twenties so struggling without the specialist guidance.
