Possibly TMI but itchy bottom 🫣

I was wondering if anyone can help.

I have been experiencing an itchy bottom for several months now. The GP has examined internally and externally and sees no evidence of piles. There is no blood or mucus either.

A dermatologist has confirmed there is no irritation or skin condition at all.

I have done all the usual things such as sensitive washing detergent, washing with water after a bowel movement and pat dry, cotton underwear, keeping the area free from moisture but nothing is helping.

Haven’t seen any worms.

There is a mild itch constantly but it’s incredibly itchy after a bowel movement. A bowel movement isn’t painful though so I doubt a fissure, I’ve also been examined many times now.

With the absence of a skin condition I have moved away from creams altogether as these have not helped - sudocrem, anusol, steroid, canesten etc and keeping the area dry.

has anyone experienced this?

Possibly a neurological issue?
There are nerves around the area that if damaged further up can cause itching and or stinging/pain.

Sounds like peri-anal excema although I’d have thought a dermatologist would have picked up on that.

Have you tried 1% hydrocortisone cream? It’s about £5 over the counter.

have you treated for worms? You don't have to see them. Would seem the obvious first step.

Do you have itching in front?
Lichen sclerosus has terrible itching around the vulva and anus in a figure of eight. It is an autoimmune condition more common than people think.

Have you actually treated for worms? Always treat anyway.
