Intense pain where cervix meets vagina and sense of tearing there on both sides, plus abdominal tenderness

Hi,

just been to the GP, could anyone help me figure out what is happening.

urine dipstick normal.

This has developed over the past three days.

She pressed my abdomen while I was lying down, and then I went and sat back in the chair I started crying with pain as there was like spasmodic pain where she had pressed and I had to lie back down again. It feels like it centres on my right ovary. It feels like maybe right ovary area has been tender for a couple of weeks. Last period started on 20/2/23.

that kind of awful tenderness is everywhere, like back passage and front passage making it hard to find a comfortable sitting position.

It is incredibly uncomfortable where the cervix meets the vagina, that’s really why I went. There’s a sensation of maybe fluid or if I had to guess an abscess or something, on both sides at that point, and if I bend over or sit at the wrong angle there is a sensation of tearing there, then the pain takes a long time to subside.

when lying down On my side, I have the feeling to keep a knee up and open, to stop the feeling of something being compressed there where the vagina meets the cervix, to relieve the pain

pain is almost nauseating now.

Has become difficult and painful to wee, smaller stream, and very frequent urgency. But urine test normal.

there is also a bit of vulval pain in the past 12 hours.

as of last night dread pooing as there is like a tearing sensation in different bits now too. Even sitting in the loo at gp to try and give the urine sample there was that tearing sensation when tried to sort of open ureter or whatever muscle is used to wee when you are trying to wee.

I don’t think there’s any abnormal vaginal discharge but I can’t really tell. Maybe clear.

I feel so unwell. She referred for a routine ultrasound that may take some weeks.

it’s hard to sit on a chair even, everything is so tender and have to shift position.

feel slightly feverish but no fever. Not much appetite.

she said not appendicitis as she pressed there.

Im not sure what to do next. Feels like I’m getting worse by the day.

is it a reproductive system thing or a urinary tract thing or something else.

I had sexual contact last 3.5y ago. Went to GUM clinic a week after the encounter and all was clear.

did not do vaginal exam as I have not had intercourse before. So have never had a vaginal exam.

as I sit here pain in right ovary region radiates to hip.

I’m going to have a little lie down just to say that here in case my replies are slow.

she gave me some hormonal blood tests and also a full blood count which I’ll try and do today or tomorrow. The hormone tests I have to do later in the Month I think.

she has given me also vaginal swabs but I am in too much pain to do it now and she didn’t think it would show anything.

she couldn’t explain the tearing sensation or tenderness and just said go to a and e if it gets worse.

i forgot to say the cervix pain radiates into my foot, that is to say each sides pain radiates into that sides foot

It sounds like the sort of pain one might experience with endometriosis. But usually that develops gradually, not suddenly.

Obviously you won’t know until you’ve had all the tests. It might be something else.

Commiserations though. Hope you’ll find some relief soon.

When she checked your abdomen did she say anything about fluid? Did she tap to see if it was there? Honestly I would be pushing for an urgent scan referral if it was that painful and your symptoms include a feeling of fluid and frequent urination. Also she should be getting CA125 levels checked with your bloods. I’d honestly call them back OP.

Thanks both. Very much. I went to a and e. It was really traumatic for a number of reasons. I did get a ultrasound scan showed a cyst on anterior vaginalnwall. 1.5cm. Basically they did not believe me and said my symptoms couldn’t be caused by the cyst and referred me to vulva sensitivity clinic with a several month weight. I was crying with pain and couldn’t walk. Had to be wheeled in wheelchair. Sensation was of ulcers where vafina meets cervix. Both sides. Pain radiating down both legs. I started myself on antibiotics out of date I found in a cupboard earlier in the day I went to a and e. They were quite sarcastic about that but gave me the same small dose in date ones amoxil 500mg 3x a day. It got w bit better yesterday but now getting worse again. I’m scared. So it’s like there’s an infection at top of vagina and it spreading upward into womb and ovary area. Is how it feels. Radiating down to my foot too. No one will listen. Thank you both for caring it means a lot. It feels like the tissue all around is infected if it makes sense. Like the connective tissue and nerve tissue which is why my leg and foot hurts. Don’t have temperature as such but alternate between shaking and chattering, and then hot sweats.

I honestly don’t know what to say to you … It’s horrific to think of anyone suffering that kind of pain and not being offered swift and appropriate help.

I'm not a medic but I'm really concerned by your symptoms and the fact you've been sent home and it seems to be getting worse? Can you walk now? Is there anyone with you?

I'm so sorry you've had such poor care. I think this is sadly one of those cases where you are going to need to make a nuisance of yourself until you get the care you need, although I appreciate that's probably the last thing you feel capable of at the moment.

So sorry you are going through this.

The shaking and chattering (rigors) alternating with hot sweats are sepsis red flags, please get some speedy medical attention

Your GP has directed you to A+E if it gets worse, so I think maybe that's where you should be heading.

Do you use tampons? Could it be a forgotten tampon?

If the antibiotics don't help I think I would go back again, you sound in terrible pain

rhursday was when I went to a and e. Friday morning I felt near normal but it got worse over he day. The shakes and hot sweats I told the doctors about, which started Wednesday night, they first said it was nerves as it was first vaginal exam, then I said it happened last night and they said it was a relation to pain . I had them again last night and this morning alone with awful pain in that space and leg and coming back into womb and ovaries. After I posted I did something maybe silly, we have metronidozole in the cupboard (family member diabetic and prone to infections and has some old prescriptions), l took half a 400g tablet and the pain is a lot better. The feverish ness subsided also for now. I am desperate not to go back to a and e. I am trying to hold out til tomorrow and maybe see a private gynaecologist. I have tinnitus which had become worse by the antibiotics so am trying to balance that. I took the metronidazole because reading the net it might be cervicitis and I thought maybe PID. The gp said it’s not as I would have discharge, and the a and e doctor said it was not because my tummy was a lot less sensitive (but still painful) than when I saw the gp which I attributed to taking amoxicillin from midnight the day I went to a and e, but she said only metronidazole would have had an effect so quickly due to the type of bacteria in PID so it couldn’t be that. Ideally I would have a antibiotic pessary which I asked her about but she said it was for bacterial vaginosis. They took a lower vaginalnswab. As not had vaginalnexam before it’s the most they could do.

it feels like none of the doctors believes where I say the pain is coming from. Abs how bad it is. Night before I went to a and e j felt in real trouble, like a sense of doom almost, which is why I took the amoxil and I took doiubke, 1g instead of 500. I was starting to have the same feeling early hours this morning.

Please forgive a very impertinent question - but we know some races are treated more respectfully, sympathetically, and promptly than others in the UK. I’d say if you’re not white (I don’t know, obviously) it’s all the more imperative that you insist - or get someone else to insist - on more urgent treatment.

Yes very well observed im not white - also with virtually no sexual history in my 40s which I’ve had to tell them to in light of the symptoms and their questions, also I think presenting as very ‘young’, my family tell me I have ASD but I don’t know and haven’t a diagnosis - I get the feeling I irritate the doctors /they do not take what I am saying at face value.

Your pain sounds horrendous, @parlancet . I can understand why you're taking anything to get some relief, but I really hope you can get some help. If the shaking and chattering return, please do go to A&E again if you can.

Go back to A&E with an advocate. Someone you know who is NT and can advocate for you. Someone formidable who won't be fobbed off.

Sounds like it could be something like one of the following (all of which I have experienced at some point btw)

  • Endometriosis
  • Burst cyst
  • Ovarian torsion (this can be very serious and can result in a dead overt and surgery required to remove it, so should be treated urgently)
