How to treat stubborn haemorrhoids (and potentially a fissure too)?

Ever since having my dc 17 and 15 years ago, I have suffered from on/off haemorrhoids. I also developed a fissure after having my last dc which used to feel like I was passing razor blades when I went to the loo!
I did manage to keep it all at bay but I have IBS-mixed which means I’m sometimes constipated sometimes it’s the other way, so can flare it up if I’m going through a constipated stage.
All made worse in 2019 as I had a colonoscopy and anyone who has had the joy of the prep before the procedure will know that it involves drinking a shed load of laxatives which will result in peeing out of your bum and sadly for me, also an exacerbation of my haemorrhoids (that was an understatement!). It then flared up again after needing a pill camera endoscopy last April which resulted in more laxatives!!
I was doing ok but I have been having an IBS flare up over the last few months, and at Christmas time in order to get through the day I took Imodium which made me quite constipated and (sorry for tmi) but when I went to the loo the next day, I could literally feel the haemorrhoid/fissure ‘crack’ open and there was some blood when I dabbed paper on that area.
There hasn’t been anymore blood but I just can not get the haemorrhoid to shrink, it is purple and hurts like a bugger. I’m getting scared to go to the loo now.
I use off the shelf cream morning and night but every day I open my bowels it hurts so much and all day too and it flares even more if I stand for too long.
I have a GP appointment tomorrow so will obviously mention it but I’m afraid to try a prescribed medication as I’ve read they cause awful migraines (I already suffer from aura migraines) and really don’t relish the idea of easing one issue only to end up suffering from another. I also have to be careful with my diet due to the IBS so can not eat lots of fibre as I’ll end up with diarrhoea which will further exacerbate the issue!!

Does anyone have an experience of haemorrhoids and fissures and could offer up any advice how to tackle it?

Definitely mention it to your GP.

The cream doesn't cause headache for everyone and there is an alternative cream if you are one of the unlucky people with headache.

echo @AnnaMagnani advice and would also add - and this felt undignified fot me when i started but it works - using benpathen (sp?) nappy rash cream. I have had fissures but am also using an immunotherapy cream for anal pre cancer which causes skin to blister and break and the nurse recommended it and it really helped

I’ve had external ones for years and nothing like anusol etc has done anything other than make them raw. I have an appointment for banding in a couple of weeks but I don’t have high hopes for them as their mostly external. What I’ve used this last week which has taken the pain away but not really the swelling, is virgin coconut oil that you would use to cook with. I just massage it in a couple of times a day and can’t believe the pain is relieved without painkillers. Bloody awful condition so hope you can sort something out.

Thanks pennee I used pure coconut oil to remove my make up so have lots in, I will try that. I do hope the procedure works for you, they literally are a bloody pain in the arse!
