Pill query - breakthrough bleeding

I'm wondering if anyone can advise please?
DD (16) started taking the Gedarel 20 combined pill in August. She was having breakthrough bleeding so it was switched to Gedarel 30. She is still having the problem. What happens is she gets to day 15ish of taking it and then has discharge with a bit of blood in it. If she carries straight onto the next packet she doesn't stop bleeding for a few weeks. If she has a break then she has a period and then when she starts the next pack she is fine.
Our confusion is this. When we saw the nurse before she changed it to Gedarel 30, she said she could stop it as soon as the breakthrough bleeding started, have four days off and then start again. I am concerned though that if DD is using it for contraception (I'm not entirely sure, but she has been with her bf for ten months now and I'm not totally naive!) then she won't be covered properly. She started bleeding again today (day 15) and she wants to have her break now, but I'm not convinced she should.
I'm going to try and get an appointment with the nurse but last time we had to wait four weeks and we could really do with some advice sooner.
If anyone knows more about how it works I would be really grateful. I've looked at the patient leaflet but it doesn't really say anything about it. Thanks.

She would.need to have been taking it for 21 days before taking a break to still be covered against pregnancy. Is she running the packs back to back or taking them normally?

She was told she can take up to four packs back to back. She tries that but would then have a longer time of bleeding, so she's been having a break.

What has confused us is that the nurse said that if she wasn't using it for contraception (and she definitely wasn't when she started taking it) then she could stop it when she started having bleeding for four days (even if day 15 or so) and then start again, but that doesn't seem right to me! She said the pill has changed a lot since I took it 30 years ago!

That does seem to agree with what the nurse said, near enough. I think we'll go with that until we can get an appointment. Thanks again.
