Endometrioma? But Dr didn’t disclose?

Hi mums. I just had a strange experience. I’ve had some mild pelvic pain recently. It started two months ago out of nowhere during ovulation. Saw a dr who did an external and internal US which showed a cyst on my right ovary and possibly a small fibroid (but not certain). He asked me to come back in two months.

Back I went today. I’ve had pain again during ovulation as well as cramping in the five or so days leading to my period. All of this is fairly new to me. I have very light and tolerable periods which are regular and last three days.

Cyst was still on my right ovary and he said it was likely functional (it looked solid black and round on US). He commented the left ovary looked fine with follicles in it (looked like a bunch of smaller circles on US). He then said he wants to send me for an MRI to confirm - he didn’t think it was anything serious though. I asked if this was routine (I just moved to a new country) and he said yes.

I later checked his MRI referral form and from what I can make out from his handwriting it says “rt. Ov. Cyst, likely functional. Lt. ov. Cyst endometrioma possible”.

I’m so confused. He said my left ovary was fine. Sending me for an MRI for a functional cyst seemed strange but it’s not strange for an endometrioma. But why wouldn’t he tell me? I’m absolutely shitting bricks thinking he’s hiding something awful from me now. And don’t you have to have endometriosis to have an endometrioma? I’ve never had any symptoms of endo, conceived two children on first try each time, and had two successful CS. As I said, light, tolerable periods and no other pelvic discomfort other than recently.

I don’t know what I’m looking for. I’m just worried about this weird pelvic discomfort I’ve had and now I feel the trust is broken with this doctor and I’m convinced I’ve got ovarian cancer or something else terrible. Is it just me or is this weird? Mri is next week but I guess I’ll have to call him back tomorrow to ask what’s going on.

For years, I have had the same issues you describe.
I get (and have for many years) terrible pain during ovulation, it lasts about a week and hurts even to sit.
No doctor or gynaecologist has taken it seriously (I have been under the gynaecologist department for years for persistent heavy periods and uterine polyps).
I have had many Pelvic US and I’ve usually always got an ovarian cyst on one or both of the ovaries, sometimes they have disappeared at a later scan, other times still there. A few years ago the sonographer said I had a endometrioma, when I asked her what it was she said ‘oh, just what we call a chocolate cyst’. I thought nothing of it but it want until my sister was rushed into hospital for an emergency op to remove a suspected ovarian cancer mass, (which turned out to be endometriosis) that I started to google. Turns out chocolate cysts are connected to endometriosis.
I am trying to seek a MRI to find out for sure although I am told the gold standard test for endo is a laparoscopy.
I am still trying to get to the bottom of my issues but believe I could possibly have endo.

Btw, if you are concerned about OC, you can ask your GP to do a CA-125 blood test. I had one earlier this year.

Thank you for your message! Endometrioma is definitely linked to endo as you say. It’s essentially endo growing on your ovaries. so frustrating that the only way to definitively diagnose endo is through surgery, isn’t it?

I’ve thought about the CA125 but frankly it terrifies me.

I'm going for my follow up scan next month for the exact same op, except my periods have been hideous for years. I was told my cyst was endometrioma aswell and that it was "blood filled". What does this all mean then? I asked and the sonographer just said "it will be from the endometriosis ". Is it cancerous?

@Harrystylestutu ii have stage4 endo with cysts so large my left ovary is no longer visable. The cysts are descibed as" chocolate" or blood filled as the are filled with the old blood from the endo. Thats normal for endo and not cancerous.

i believe they’re not cancerous in and of themselves but I’ve done a bit of research and apparently but they can become cancerous though it’s rare. That’s why they’re monitored and large ones removed.

Im mostly worried why my doctor didn’t mention it to me. It’s very odd. I wonder if it’s cultural but surely not (Japan)

Thank you so much guy, @Fadeout83 @Redannie118 . I guess I'm kind of lucky (?!) because I've had all the kids I'll have after hideous pregnancies and births left me really ill.

so sorry, this is embarrassing and gross, but the sonographer said from my symptoms I have rectal vaginal endometriosis, because my symptoms start a few days before in "that place" and it's just like a heavy period, clots, flooding etc.

I thought this was just endo as my mum and gran had endo, but never had treatment, so I thought these were normal heavy periods 😞 had the same symptoms, but I'm constantly in pain. Either lower back, sharp stabbing up, intense aching on the right where the cyst is, so much I throw up.

I got told off as I've had these symptoms for 15 years, but just got on with it. I can't take hormonal contraception,even though I'm now sterilised, because it made my blood pressure go stupid high. I don't want a coil, as surely the endo will just encase that too? my cyst is 3.7cm.
