Hi. Long-term poster (over 11years) but N/C’d for this as potentially outing.
At the start of the week I chopped off the end of the tip of my index finger (the top left corner, including half the nail). I was able to pick up the severed bit from amongst the onions. It bled profusely for a long time, so I held it above heart level, applying pressure with sterile bandage. Once the bleeding had calmed down after a couple of hrs my DP (nurse, albeit in MH) applied a blister plaster to ensure it had a moist healing environment & didn’t adhere to any dressings. He then wrapped it in sterile bandage and we put a finger stall over to keep it dry & clean. I’ve had it like that all week.
Yesterday I noticed it starting to smell 🤢 and today the smell was horrendous.

So earlier today I asked my local pharmacist for advice & she suggested minor injuries for them to look under dressing & re-dress. I wandered up there & once I was seen the nurse there removed dressings, washed it a bit, didn’t seem phased by the smell, and said it was hyper-granulated (prob as a result of the hydrocolloid dressing having over-stimulated the normal wound granulation). She cauterised it using a silver nitrate stick (hurt like HELL) and said to leave air to get to it on walk home then put normal plaster over which I’ve done.

My Q is: should it be swollen, and should it still stink? Is this a normal part of healing or could it be infected/necrotic as well as hyper-granulated?

I’m loathe to seek further RL support seeing as nurse seemed unfazed, but then I didn’t mention the fact it’s a lot more swollen than the one on my other hand, and she didn’t smell the actual wound (she said the smell was the dressing). I’m not sure if I’m just being overcautious now I’m home with a persistently stinky finger! 😥 🤮

@Dotcheck I’d already been before I wrote
my OP. I was there all afternoon today, but it’s since I’ve come home that I’m doubtful whether my finger should still be smelling and swollen.

Is it the finger that smells? The dressing will have smelled. It doesn't look red or swollen. I'd leave it be.

Replying here from someone who had a severed fingertip. Mine was severe though and I spent three days in a plastics ward in hospital as I also had a broken bone but feel qualified to answer 😊.

I would expect swelling. You've had it looked at and I'd just keep it clean and dressings changed regularly. The swelling will reduce. You don't need stitches which is the main thing.

I don't think it should be green. Working in a surgery I've seen wounds like this and it was gangrene. But then there's no redness around yours. If the smell was the bandage then it should of stopped after it being took off. I honestly don't know but hope you get it sorted.

Go back again. I would for peace of mind. Tell them it’s just swollen up and is sore. Get a second opinion. You done want to mess around with a green wound.

Oh and obvs don’t let your DP mess about like that again.

I did similar but didn't cut it all the way off, so it was a flap held by a tiny thread. I strapped mine down with steristrips then covered with a burn dressing. Mine reeked as well and was swollen. I think maybe 5 days later I popped into work and asked one of the community nurses to check it because I was a bit worried about the whiff but she said I'd done a good job and it was healing well. She soaked off the steristrips and re-dressed it. I got some silicone finger covers from Amazon.

Mine never went green though! Hope it heals soon.

Keep an eye on it. I'd be inclined to get the GP to look at it if smell doesn't go, or if it is more painful, hit, red, swollen or you start feeling systemically unwell. Is your tetanus up to date?

The silver nitrate is to kill off the over granulation so that's why it's black. It's meant to die off.

The surrounding tissue looks ok (not red or angry) so I'd be happy to leave it if it were my finger.

Just keep it clean and covered.
