Does this wound need looking at again?

Ds2 is 11 and sliced his foot up badly yesterday.

Minor injuries looked at it and while one of the cuts was straightforward, the other was jagged, deep and bleeding heavily. They put 2 stitches in and bandaged heavily to help stop the bleeding. As instructed we removed that bandage today and put a fresh dressing on. I was surprised at how wide the cut was and the stitches didn't really seem to be pulling the edges together. My experience is around nice clean surgical wounds though so don't know if this is actually normal for a trauma related wound. DS has a school trip to a theme park and his y6 residential next week so is desperate to be on the mend by then.

Can any medical types confirm if this is all normal looking or if it needs looking at again? I want to give it the best possible chance to heal well. He's awaiting an asd assessment and did not cope well with the hospital so would rather avoid putting him through another session of someone poking his foot if it looks like it should do.

I'm with Hash Brown and have tissue viability experience. Is an infection risk, but because wound is less than 48hrs old and still in inflammatory phase and your boy is feeling well, it shouldn't need full systemic antibiotics yet, could be treated properly with a topical antimicrobial such as silver. Precautionary abx are given only in specific circumstances nowadays because of antibiotic resistance. But this will largly depend on how good the practitioner is that you see. Either way I wouldn't submit him to a&e tonight. Minors should be OK tomorrow unless he becomes unwell. Pain killers overnight. Hope it heals quickly!

Ouch that looks flipping sore
if it says s deep does it need to be packed with a dressing?

Agree!

My goodness yes I’d get that looked at, poor thing!

They really shouldn't have advised you that it was ok to walk on that. Walking on tip toes is obviously going to stretch the foot and pull the stitches and skin apart. Whoever gave you that advice has caused this. Terrible advice. Poor lad. Hope he's ok soon.

Good God I was not prepared for your photo. It looks very poor for a wound of that size. Good luck tomorrow.

Agree with this. Worth asking GP surgery if one of the practice nurses would see it tomorrow? Though it's only a few days old still so minors would also still be appropriate. Our minors unit usually tell them to come back to GP for follow up, but they don't mind seeing things again if needed.
I agree with PP that telling him to walk on tip toes wasn't great advice, but it is a very awkward place to cut, a boot wouldn't necessarily help.

I hope you get help and your son is better for his school trip x

Now, I’m the sort who would need to be approaching ‘the light’ to drag myself to A&E… but even I’d take the kid with that gaping wound.

Poor boy, that looks very painful. Definitely needs checking over again I'd say.

Thanks everyone for your help- went back to minor injuries and she agreed the stitches weren't actually holding the wound closed so took them out. She said it had started healing so didn't want to interfere too much with it. She gave it a clean, put something across it to help pull the sides closer together and put a big thick dressing on it to give it a bit more protection.

We've got to go to the practice nurse on Friday to check it is healing. DS2 was very brave and negotiating for a shopping trip to the Warhammer store as a reward.

Ah great, think he deserves a bit of a shopping trip after all this ! And maybe a glass of vino later for his mum too ! 😁

DiDonk · 12/07/2022 14:43

Crikey, dd had 6 stitches in a smaller wound following a bizarro failing over in a shop accident.

The doctor in a&e said she needed so many otherwise the edges would pull apart

Glad to hear he's got it on the mend @BobBobBobbing

I was under the impression we don't stitch feet but I could be wrong.
