Nail psoriasis help!!

My nail psoriasis has significantly worsened over the past 2 years. I suffer from onycholysis (nails going white and then falling off) on my finger nails despite keeping my nails short and moisturised. My toe nails on my right foot are misshapen and have subungual hyperkeratosis. Calcipotriol doesn’t work.

I also have plaque psoriasis, but this is easier to treat when I have a flare up.

What have you tried? I used to have the most beautiful nails :(

To be honest, the only thing that's made any difference is anti TNFs/biologics. My nails are in much better condition when I'm on them.

I used to have this, then I got a surprise diagnosis of being coeliac. Obviously completely different autoimmune condition I know BUT surprise revelation was that all plaque/scalp psoriasis and most nail psoriasis cleared up.

Conversely, years ago I realised that Tom yum paste (random but I used it a lot in cooking) was a huge trigger.

I don’t think enough is known about what food triggers psoriasis if any, but something like Whole 30 might help you identify any triggers?

I don't use anything for my nail psoriasis. I haven't ever pursued it because it didn't cause me as much of a problem as my plaque psoriasis on my hands etc. Going to follow with interest though. Sorry that I can't offer any solutions. It sounds like yours is quite bad. I used to love my nails too. They look awful now. All lumpy and hard. Flowers

@Gardeniafleur I was diagnosed with coeliac nearly a decade before I developed psoriasis Sad @milski yes my nail psoriasis is horrific. Ive lost 2 finger nails and it looks like I’ll be losing another soon. Right toe nails are awful too. @mynameiscalypso I haven’t heard of those treatments.

@stopthepain I used to have terrible nail psoriasis in my toenails and going to a podiatrist was a game changer. She could cut them short and thin them so they looked a reasonably normal shape and then I’d go to the salon and have a gel pedicure in the summer. Silly things in the grand scheme but having my toes out in summer felt amazing. I now take the biologics so my psoriasis has cleared, but whether or not you’re eligible for them depends on the extent of the disease. Do you see a dermatologist?

Thanks @PinkTonic what were your toe nails like? Yes I see a dermatologist, but nothing has worked so far. My nails are getting worse and worse. I see her again in a couple of weeks.

Until I went on a Biologic for my psoriasis nothing worked on my nails. The dermatologist also had no magic cure for nail psoriasis.
After being put on adalumimab it took about 3 months and for the first time in years my nails looked almost normal! 2 years down the line and they look ok but still do tend to sit or break off quicker than they should

@stopthepain they were thick misshapen lumps, impossible to cut. Underneath the nail, skin would build up and cause pressure on the nail bed which really hurt. They were horrible. My fingernails never got as bad although I had pitting and they would lift away from the nail bed to about halfway. None of the topical treatments do anything for nail psoriasis. You can ask your dermatologist about biologics. How you feel about your nails and how this impacts on your quality of life will definitely factor into eligibility for the drugs.
