NC for this for obvious reasons...

So for a couple of months now, my anus keeps getting sore and itchy, often a bit worse when I've been to the loo, and I think there's been a couple of tiny fissures at times which hurt but then heal quickly. It feels like it's burning a little bit internally sometimes but then the feeling passes. Nothing visible on the outside, no piles or anything.

I know I have HPV as confirmed positive at my last smear test. I'm now freaking out and thinking I have anal cancer. I'm quite young- 30ish- is it that likely? Or could it be something like internal warts (gross).

I know I need to go to the doctors. I can only go if I can see a female doctor though.

Should I go to my GP's or to the sexual health clinic in case it it warts or something? I know I sound fucking stupid but I don't know what to do and thought of the sexual health clinic too as I've found them to be great.

Are they going to be able to take a proper look up there, there and then? Do they have the necessary 'equipment'? Or will I have to go to hospital? Again this is pathetic but the thought of a doctor looking at my arsehole is horrific and I'd be delighted if I could hopefully go through it on just the one occasion.

I don't know why I'm posting, just feeling scared and perhaps needing a kick up my broken bum. If anyone could reassure me that it's likely to be something far less sinister that would also be fab.

HPV is transmitted sexually yes and some strains can cause genital/anal warts. And cancer, although most commonly cervical cancer. And I know my body never clears HPV, I test positive at every smear.

I thought the GUM clinic would be more use than the GP if this was what is actually causing it.

Thrush can give you vulval as well as perineal and anal pain and itching. It’s driven me to distraction before. Confirmed by a swab. The gp was shocked at just how high the reading was.

It sounds like fissure to me. Go is first point of call. They get you to lie on your side and they look that way, it's not like bending over or anything. If they tgink it's a fissure they won't go internal. If you get forwarded on to hospital, it will be to colorectal team, so the doctors there literally deal with bums all day long, it's there speciality. To put you mind at rest.

I'd never thought about thrush- is that possible?

I actually had bacterial vaginosis a few months ago, and that was kind of around the time I first started getting problems with my bumhole as well. Sounds so stupid but is there any way of the same bacteria that causes BV causing problems with your arse as well?!

Firstly, calm down - it’s unlikely to be cancer or anything sexual health related. You need a GP not a GUM clinic. If not PA, my money is on fissures. If it’s fissures, keep everything regular and soft to give them a chance to heal (movicol, orris fruit cubes, water, good diet - not too much fibre). You might need special cream (diltiazem is the best of two as the other causes headaches) to help the muscles relax in order to heal.

Also - if it’s piles, Anusol suppositories are the best for providing relief. You can try them without waiting for diagnosis.

Unfortunately, I’m a bit of an authority on arsed. Had a fistula laid open last year after 20 years of fissure problems.

Here's some general bum advice I go by..

Drink more water. It helps with elimination. It literally smooths it and helps it leave easily. Ever constipated or getting piles or cuts? Drink more water.

And...consider a squatty potty. I heard of them but didn't have one. Where I lived once there was a small step ladder in the bathroom so I decided to try putting both feet on it, as per squatty potty instructions, and it made a difference. Feet up makes the colon a straight line hence easier.

I don’t think BV could exist outside the vagina…

A friend of mine went to a doctor and declared that she had ‘bum thrush’ when he asked what he could do for her. Apparently, once he finished wiping away tears of laughter, he told her it wasn’t a thing.

Definitely get it checked out, but it's most likely not something serious like eczema or psoriasis, so try not to worry

Thank you for all the advice, it's helpful trying to consider the other things it could be!

I tried bamboo loo roll recently but that made no difference.

Using ph neutral soap made no difference.

I wonder about fissures but I know when I've had tiny ones in the past (literally tiny, how within a couple of days type things) they have stung a bit when going to the loo but the burning feeling hasn't been in the inside of my arse.

Piles- it could be- I've never knowingly had them in the past. My arse literally feels on fire atm, it's so bloody uncomfortable 😣

@OhFFSwhynow I could have written this. I've tried anusol suppositories. Go prescribed steroid cream for internal. It helps but doesn't make it go away. Mine is worse if I've had alcohol and can make the external area around my anus really red and sore.

It's so weird- never had any problems like this ever- I had BV and possibly thrush before that in the late summer/September and wonder if the antibiotics I had for BV has caused this as a reaction somehow, although doesn't seem likely.

Externally it looks completely normal- it's so weird.

Try drinking 1.5l of water each day, and eating more fruit and veg for a week.

This will allow soft bowel movements so your insides won’t be having to strain or stretch when you go to the toilet. After a week the itching should go. It’s itching because inside you’re swollen and trying to heal. You will just have internal piles. They itch like crazy. The soft diet should allow your fissures to heal too.
