Can anyone help me try to understand what is 'normal' and what may be something we should be concerned about?

To set the picture we have an 90 year old man who, until a year ago was still going out and about most days independently. Covid changed that and by the start of this year we could see a lot of changes that made us query dementia. A head CT was done and showed nothing other than 'normal signs of ageing' and an appointment with the memory clinic came back 'no concerns, review in 18 months'. He was still happy to go out 2/3 times a week with us in the car, just for a coffee and a wee walk at the beach or something but now, he won't come out at all. He is sleeping on and off the majority of the day and I don't know if this is normal when you are older or if we should be contacting the GP again. B12 (he is on 3 monthly injections) and thyroid have been checked and normal. Im not sure what i am looking for here, I just needed to note it down tbh.
