TMI - chronic blood when pooing

Sorry for the gross topic... I've been having blood when pooing off and on for a few years, usually 1-2days a month. I have had a sigmoidoscopy and a colonoscopy about 4 years ago which totally clear (including for chrons and celiac). But the bleeding still happens! It's never much (Maybe a 50p sized area on the loo roll the first couple of wipes and pink/reddish toilet bowl water). Blood is "on" the loo not "in" the poo... No other symptoms, weight is stable, no change to bowel habit, no anaemia. Sometimes it is a bit sore on "exit" but not always.

The doctors don't seem to have an answer what this is, and no real appetite to find out having excluded cancer etc... does/has anyone had similar and actually ever found out why??

Hi, I have this but only ever just before my period is due. The blood is vaginal not rectal. Sounds daft, but do you know where the blood is coming from? I wasn't sure for ages but if you put a tampon in it is obvious. Apparently if you are low iron, your body holds onto your last bit of period from the previous cycle and in a fit of energy, it releases it a few days ahead of your next period. It's super annoying as I get caught out sometimes, but apparently it's not sinister. If you are up to date with smear and not overweight, I am told gynae cancers are not a concern. If it's definitely rectal, and sometimes pain on exit, I'd say it sounds like a fissure or piles? Also, if it's bright red, then it's a tear near exit, whereas if dark red/ blackish, then it's from somewhere more internal.

Hi @Mythreeknights, definitely rectal - I only have periods once every 6 weeks (run pill packs together) and it's also more like thin blood from a cut - not like (gross, sorry) mucousy period blood...

I'm wondering if it's a recurring (small) fissure, as it's bright red and sometimes a bit painful... I have slight hypermobility which means I bruise/tear skin easier than some, maybe it just keeps tearing? Sad

It sounds a bit obvious, so you've prolly covered this with the doctor, but could it be piles?

I would say piles or a fissure, I had them after childbirth so it sounds very familiar

I had an anal fissure after childbirth and needed GTN cream to get rid of it.

Not a medical expert but if anything sinister has already been ruled out and it's not all the time and at times painful/sore, then I would guess a anal fissure.

Your thread title makes it sound like you're losing a lot of blood.

The thing is when I did get investigated (colonoscopy etc) they didn't actually find a specific cause, they just ruled out the very serious causes... But they don't seem that bothered now those are ruled out even though the problem persists!

@mistyminge2 I meant chronic in the sense of "going on for a long time in total" not in terms of each "event". I never lose much blood each time it happens, it's more that it still happens 1-2 days a month for well over a year!!

I think realistically it is either piles or anal fissure which keeps opening up/coming back, I just wondered if anyone else had this pattern of bleeding 1-2 days a month and an actual diagnosis of why it was happening - I seem to have more of a diagnosis of what is NOT happening if you see what I mean!

It definitely sounds like an anal fissure to me. Like any cut or area of broken skin, they can reopen if not fully healed (or just because they're a weak spot).

Fissure blood is usually bright red and coats the poo as it comes out. I've heard that 'serious' blood is dark and makes your stools black (know from Operation Ouch that dead blood cells are what make our poo brown, so presumably you just have a lot more when poo is black).

I wouldn't worry but drink a lot more water and take steps to soften your stools instead.

If it were me , I ask for further investigations .
I can sometimes hear blood dripping into the toilet when I go. It is bright red , ie fresh blood. It is from an internal pile . I do have regular check ups.
If you have black ,sticky looking poo , that could be a sign of old blood so more concerning .
I am of the mantra, if you are worried , they worry your Dr ! ( By that I mean make an appointment )

@Sunnyday321 I would tend to agree Smile The issue is that when I asked previously "well I'm glad to have ruled out cancer/chrons/diverticulitis etc but what IS causing it" the response I got was basically " well probably nothing serious so just ignore it".

Maybe I need to push harder, get only my GP isn't at all fussed! Certainly for the last year it's felt the risk from covid from going in for more investigations seemed higher than the risk of ignoring it (given GP opinion and the fact I have had a colonoscopy already)...
