Atypical facial pain- anyone familiar?

Last August I got a very mild toothache. My dentist did an x-ray, and said that I had a little bit of decay under an existing filling. She cleaned it out and re-filled. The pain got worse. By this point I had a really deep aching and burning in the tooth. My dentist carried out a root canal and the pain got worse again. She was baffled, and said that the rct has been textbook, and she could not see any reason for my ongoing pain. Eventually she extracted the tooth, when I couldn’t cope any more. The whole episode lasted about 9 weeks, and I ended up with a partial denture to replace the (visible) tooth, just before my wedding.

I started to wonder whether I had caused the pain, by clenching and grinding in the night, and whether the pain had been due to that, rather than an issue with the filling. I knew I had some history of this, because I had ground one of my Canines flat, but it had never caused me pain before. I was given a soft mouth guard, which I have worn religiously ever since.

Since the extraction, I started to experience all sorts of different aches and pains in various teeth. In March (Just before lockdown), I had an adhesive bridge fitted, to fill the gap. The whole area of the bridge has now ached ever since it was fitted. It is exactly the same deep ache and burn that the original tooth caused, now apparently focused on the tooth behind the original one that was lost. The pain can be manageable, but some days it feels searing, and spreads across the upper cheek area in a line. The bridge has been adjusted numerous times with no effect.

My whole mouth feels wrong. My teeth don’t feel like they fit together properly. I have had aching and pain every single day since last August. Countless dental appointments. I feel in despair. I want to have the bridge removed, but that will only leave me with an ugly gap, and needing to have an alternative which will no doubt be just as problematic.

I had it adjusted only this Monday, but still the pain remains. My dentist x-rayed the whole area, and could find no issues.

I have paid to see two private dentists during lockdown, and both just said I needed a mouth guard. No one seems remotely interested.
The pain and experience I have had seems to fit perfectly with a presentation of atypical facial pain, and there are whole communities of people on Facebook who have had similar experiences.

I don’t even know who to speak to about this any more 😔

Please help!

I totally undertand you so basically I have pain on my first pre molars uppers so the molar next to my canine either side. 10 years ago a dentist gave my left side a root canal I still have no idea why but they did and ever since then I got pain from this and then the exact same tooth other side also. The paid is like a throbbing pain and I basically begged my dentist to take the tooth out on the left and take the root canal out on the right side so I have the teeth removed I felt relief for about a day and then the pain come back. It’s described as phantom tooth pain. Pain but I have no teeth there. My dentist told me it’s called atypical odontalgia (google it) also phantom tooth pain. People have been known to have teeth removed like me then remove the teeth next to it trying to hunt the pain for relief. Mines a burning throbbing pain. Now I have bridges either side and still have the feeling of pain like there’s a tooth there even tho there’s not. I’ve had mri scans X-rays etc and theres nothing showing. It’s all down to nerves. So I started taking the tablet and I brush my teeth with a sensodyne tooth paste for gum relief (home bargains) and also I use a mouthwash I swear buy it’s expensive and doesn’t last long it’s called peroxyl mouth wash it’s about £5.30 (home bargains & Tesco) the tablets do work I stopped taking them to see max at present I have pain again but I started taking the tablet yesterday takes about 4 weeks to start working. The tablets help with nerve pain a lady who has same as me was recommended it by the liverpool dentistry who told my dentist about it. Also but I haven’t tried. Rinsing your mouth in tobassco sauce yes the hot sauce! I bought some but haven’t tried it as the tablets worked for me. Whats your pain problem exactly ? Send a message insane but these tablets and the combination of tooth paste and mouth wash and flossing have helped drastically

Thanks a lot for such detailed answer!
it’s totally my story , sounds all so familiar.
about 3 years ago I started to feel pain in my front upper tooth , I did everything possibly to keep this tooth but pain was extremely strong and I asked my dentist remove a tooth. I don’t remember did I feel any pain without my front tooth, but I think mostly no, without tooth I was pain free. So after that my dentist tried to put implant but it’s failed many times and bc of so mush surgeries my 2 another teeth’s near remove tooth started to be loose and dentist remove them too, unfortunately. So we decided to make the dental bridge, at the first day then dental bridge was installed I started to feel pain in my front tooth ( or like the front area of the dental bridge) , that was weird feeling, I thought it’s gum hurt so I asked my dentist to make sure that dental bridge doesn’t touch my gum in the painful place. My pain was sometimes manageable and sometimes I thought I can’t handle it anymore …
aching, constant pain. As if there is an infection in the gum. As if the real tooth hurts a lot. I can't explain this pain more clearly. at night it usually doesn't hurt so much and I can sleep, but as soon as I wake up, the pain wakes up with me.
I dot checked , did 2d X-ray and 3D X-ray. Found 2 root canal infection. One is still need to treat bc it’s a hard one and I do not have anymore money for private dentist , so need to wait my turn in public area dentist. I have diagnose - Trigeminal neuralgia, but my doctor said that it’s not like exactly Trigeminal neuralgia it’s more Atypical facial pain.
i had a lot of troubles actually believe in this diagnose bc I thought all the time that something wrong with the bridge . I thought I want to take bridge off but now I am not sure anymore…

I live in Finland and some of the things that you recommended can’t find or I think even order here, Colgate peroxyl mouth wash is not available but I think I can try find something familiar or something with the same Ingredients.
sensodyne tooth paste for gum relief we have it here, but do you mean SENSODYNE Rapid relief
or some another SENSODYNE for gums? And never heard about tobacco sauce, but would like to try.

Bless you! Ahh so you’re not in the uk can you get the tablets at least? Yes people don’t believe you it’s horrible isnt it like it’s a mental problem I was offered therapy once because of it. The pain is very real and what makes it the worse pain of all is you cannot see the problem to fix it. Its definitely nerves teenie tiny nerves that causes it. It’s sensodyne sensitivity and gum it’s called. No whitening. I really would try the tablets they have been a game changer for me. Have you googled them and other benefits of the tablet also. Send you my love. Mine doesn’t bother me when I go to sleep. Spicy foods and smoking or vaping triggered it even more for me. I’ve not smoked or vaped for 2 years now and I don’t really eat spicy foods. I find I rub my tounge over the bridge a lot which doesn’t help. Don’t remove your bridge it’s pointless you still have the pain but will look worse as no teeth. As much as the pain hurts having the bridge removed wont fix it even though weirdly my brain must think it’s the tooth but the tooth isn’t real it’s fake. The tooth has gone (phantom tooth pain)

So Yes, i researched little bit more about this pills and the more I found the more confused I was. Bc I breastfed a child he is 1 year and 7 months- big boy , but he is for sure not ready to stop breastfeeding. I already took a big risk bc I am now taken gabapentin and noritren. So I read some reviews that from this pills some people experience hair lost, so I got little bit scary to take it.
colgate mouth wash can’t buy in my country but tooth paste is easy available.
my brain is kinda trying to blame the bridge too. It’s hard believe that I have this stupid sickness that feels like almost nothing can be done.

Butterfly022 · 14/10/2024 17:24

Ooo I have no idea about hair loss if anything when I googled it it says prevent hair loss. Yes I wouldn’t take anything if I was breastfeeding. The dr gave me gabapentin but I stopped it wasn’t for me the side effects for that drug is a lot and it’s very addictive. I’m with you when you say nothing can be done it’s horrible isnt it. If you ever do try the tablet honestly has been a god send for me they’ve kicked in again now and for the past 3 days I’ve had zero pain and I will be on these for life now for sure!
