Should I come off the pill for surgery?

I’m having an ankle operation in about a month, and currently on an oral contraceptive pill (microgynon). I was sent all the paperwork in preparation for the op today, including a leaflet on DVT risk, which included a suggestion that women on the combined pill or HRT might want to consider coming off this 4 weeks in advance, if advised by their consultant. I’m not seeing my consultant until a week before. I can get advice if I need to, from consultant or GP, but wanted to just get a sense on whether this was something that was typically recommended or not.

My main reason for not coming off temporarily - one reason apart from contraception for me taking the pill is that I am underweight (BMI bang on 18, weight periodically tracked by GP as it has been less) and the pill seems to add on a very helpful 3kg for me. I know being underweight carries a little more risk with general anaesthetic.

Is the pill a negligible risk for DVT, or should I look to get some advice about it now? TIA.
