Help with Blood results please.

Hello,

I was wondering if anyone can help with understanding my blood results. I have Hashimoto's but not all my symptoms fit within the Hypothyroid category

I will list the ones that have abnormal next too them.

Thyroid Function
Serum TSH 10.1 0.3-4.20mU/L
Free T4 19.1 12.00-22.00pmol/L
Serum T3 normal at 5.3 3.10-6.80pmol/L

Serum Cortisol 250 nmol ( wasnt taken at around midday)

Serum ANA postive 1:80 Nucleolar

Serum iron level 12 11-36.00umol/L
Serum TIBC 83.2 53.00-85.00umol/L
Serum ferritin 18

Transferrin saturation index 14.4% 20-40%
Serum B12 128 160 -925ng/L
Folate 3 2.90-50.00ug/L

I have been referred to Gastro and Endo
Im not sure if thats who i should be seeing? I have been unwell for around 6 years.

I also have had a intrinsic factor test show i am not producing any.

Hi there, I’m sorry no one has responded. I think that your ferritin & tsh are way out..low ferritin (although you haven’t added the reference ranges, I’m assuming it’s a standard one & anything under 40 is low) a tsh of 10 is way too high, no wonder you’re not feeling well! I’m not sure of your cortisol reference ranges are ok, but if they are low I’ve read that they need to be ok before the body can accept any thyroid hormone as per stop the thyroid madness info. I’m in no way a medical bod but I’ve suffered with v similar issues so my knowledge is based on what I’ve read. I’m certain the endo will no doubt put you on thyroxine, so take it from there & see how you go. Good luck!

i have taken vitamins prescrided by Gp and still my levels stay low maybe that is the reason for the Gastro referal?

The Cortisol ref range is for 8-9 am my bloods were not taken until 12-1pm Gp didnt mention anything about them but the print out says they are abnormal?

Sounds like you may have absorption issues. Have they checked you for coeliac?

@JollyGiraffe i was expecting B12 injections to be prescribed as i have low intrinsic factor so wouldnt be able to absorb it. Instead i was referred to Gastro Drs but my appointment is still months away as i was put on a waiting list.

Your B12 is way too low, probably Pernicious Anaemia if you're not producing intrinsic factor

I have B12 injections now and feel way better

That's not good. Well, the referral is good, and good that they are investigating, but if you have pernicious anaemia they need to treat you for it! And injections is the only way.

Do you have any symptoms of B12 deficiency? I think you should contact the GP again and ask what the plan is for treating the deficiency.

Severe deficiency can be really nasty (unfortunately I know first-hand!)

My symptoms
Pins and needles
Palpitations
Diarreah
I find it painful to walk sometimes.

Lots more things going on but i think most are due to my hashimotos?, well mainly any symptom i go to the GP with i am told is down to my thyroid but i am not so sure.

Are you usually referred to Gastro for Pernicious anemia?

If they think pernicious anaemia is the cause, then a referral shouldn't be needed unless they think something else is going on eg. absorption issues. Otherwise, you will just require 3 monthly B12 injections for life.

Here's a link to current NICE guidance on B12 deficiency. Pins and needles and pain when walking would be classed as neurological effects and do require prompt treatment.

I do remember the GP saying that she was contacting The Gastro Drs as although i have low B12 i am not actually Anemic.

But i guess if i cant absorb B12 i would eventually end up Anemic?

Do you know if they did a blood film?

Vitamin B12 and folate deficiency cause megaloblastic anaemia, which is different to iron deficiency anaemia. So your haemoglobin levels could be normal (as were mine) but can have a different type of anaemia caused by lack of B12 and folate.

No i dont think that was ordered as she just said that my HB was Normal.

A blood film is where they take a smear of blood and put it on a slide to look at under a microscope, to look at the types/shapes/sizes of the different cells or components of your blood.

@JollyGiraffe thank you, You were right Prenicious anemia.

I had an appointment with Endo today and the Dr has given me a prescription for 1mg B12 injections 3 x a week for two weeks
5mg of folic acid & upped my thyroxine to 175mcg daily.

I have also been sent for a Endoscopy appointment this sunday 😓 my gag reflex is awful and im really worried about it now.
Looking if i have celiac disease.
