Sore arse (bloke), apologies if TMI but help appreciated!

Read and respond to lots of threads in gen health and mental health, so hope that qualifies me to ask for some advice/identification/wisdom/hilarity, etc., despite being a man (last time I checked).

Basically this started in March when I had a grim virus that wouldn't let up for aaaaages. Thought it was piles (still not entirely sure that's not part of the problem). Have had them a fair few times over the years, even had one banded when I was 30 (51 now) but usually they go away with cream or gel and time. Not this.

No bleeding or even spotting that I've noticed, and I've been pretty vigilant. Work with children a bit and child-mind too for friends, so took Ovex in case it's threadworms, but to no avail. And anyway, it's not itchiness, it's just a kind of burny soreness right on the hole, the external sphincter to be more technical. Sorry if this is grim reading!

Feel it on and off throughout the day, the discomfort. When I go for a number 1 or 2 (I sit down for both, makes keeping the toilet clean much easier!) I often feel the soreness increase on both sides of the - ahem - chocolate starfish, but mostly externally. Not even getting any shooting pains in the rectum with this anymore, although had a fair few of those when it started in the spring. Like I say, it's more the sphincter itself feels tender, a bit burny and achey. Diet pretty good, lots of fiber and very little processed food as a rule, drink plenty of fluids, teetotal, non-smoker. Haven't had sex for over a year and got fully tested after the last time (don't ask). So this isn't the result of a good time, more's the pity.

Been to GP twice with it, saw a different one each time. When I saw the first one I was also peeing far too frequently and urgently, but that is no longer an issue. He stuck a finger up very unceremoniously and said he couldn't feel any piles but there might be a small fissure. Prescribed something that was a cross between thrush cream and hydrocortisone, but it was useless to me because I can't use steroid creams or ointments (long story, but if you've heard of red skin syndrome you'll know where I'm coming from).

Couple of weeks later went back and the other GP referred me to Guy's, colorectal department, but the appointment isn't until the twenty-somethingth of next month, and this is now really getting to me. Have gone between using witch hazel with clove oil and/or tea tree oil, canesten 2%, sudocrem, anusol, prep H, germoloids cream. Nothing seems to touch it, really. My arse aches with it some days, the buttocks I mean, and can feel a bit 'cold'. I've googled the symptoms until I'm blue in the face (to match the cold arse cheeks) but all I get is stuff about fissures, pruritus ani, piles, etc. and none of them match what I'm experiencing. I can't actually see that much, suspect my ring looks a bit red but it feels (to my fingers) not too swollen or piley. On and off get quite an upset tum with it, and I generally feel quite tense and anxious in my abdomen with it, difficult to switch that off or relax it much. Lots of wind at times (my neighbours must be horrified and thinking of moving by now; in fact, I'm surprised I haven't been reported for noise pollution.) No constipation or dia-whatsit (can't be arsed to spell that). Oh, one perhaps significant detail: it often feels like inside on the right is particularly achey, in the rectum, like a dull pain that comes and goes. But all the reading I've done about fissures says it hurts to poo, and I haven't had that. Or bleeding, as I said earlier. And definitely not itchy.

I might be on a promise this week and so am gutted that it hasn't resolved itself by now! If anyone has any insight, or even just thinks this is amusing and can respond in kind, please feel free to post a reply! After 5 months of this crap I'm losing hope of ever feeling 'right' back there again Sad

You need a sigmoidoscopy (camera up bum Grin) which they may do at your appointment. It's not too bad, they only go as far up as is comfortable and if they need to look further they'll have you back for a colonoscopy as a daycase. But I don't think you should wait a month- I would ask the gp why you're not being referred on the two week pathway, just in case of something like a tumour in the rectum. Much more likely to be something easily treatable though. Good luck!

I agree.You be had frequent pe eing too.Go back please.Also come into Macmillan cancer support..There is a section on anal cancer......don't want to scare you but the patients talk about symptoms without embarrassment.I'm on there as both parents had advanced cancer.I hope this is something minor for you but please bang on at gp until you get the answers.Good luck.

So sorry I haven't updated before now, thank you so much to all those who've replied to my post so helpfully and caringly [hug]

What a week. On Tuesday I panicked and went for a full sexual health screening, despite not having had sex since last being tested a year ago, haha! I dunno, just thought I could have something of that sort that had been too dormant last time around and they mightn't have picked up on it. Anyway, I got the sweetest and most supportive nurse ever, she listened carefully to all my concerns and then did various tests and had a good look at the ahem outer posterior region to see if my paranoia about possible genital warts had any basis in reality! Thankfully she said the skin looked completely healthy apart from one very small purplish bump which might be an external pile, but she said it was tiny. So I was relieved there was nothing to see, and she swabbed the old back passage for good measure. My 60 second HIV result was negative (not that I thought it wouldn't be, until she poured the blood onto the pad and was waiting for the blue dot, and I suddenly felt very anxious just in case ). No syphillis either, phew.

I'll get results for chlamydia and gonorrhea next week, but I'm sure they'll be fine. And actually, since seeing her all I've been doing is wiping with a damp cotton pad after using loo roll, letting things dry and then applying a touch of canesten externally, and most days the discomfort has hardly been there and the internal pain has been much improved.

But then on Thursday morning I broke a toe Shock and the resulting tension of holding myself differently to get around as painlessly as poss did bring back the internal pain for a couple of days, but by today it has really calmed down again. Obviously I won't know until next month at Guy's, but I feel more positive now that it's either a tiny fissure or an internal hemorrhoid that pops out occasionally and causes the pain intermittently. Since my mobility is now very compromised (did my first ever online supermarket shop on Thursday evening lol) I'm going to hang on for the appointment rather than try to get it brought forward. Will keep you posted.

Meantime, if anyone wants to join in with their own tales of arse woe, please feel very free. I've found this thread gave me a huge boost, just to get a few caring replies. It makes such a difference Smile. Thanks again!

Like I say I hope it is a minor thing.Too many people on macmilla n wish they had sent their families back to the GP when silly niggles didn't clear up.Both my parents were diagnosed at a late stage as symptoms were vague.Any bleeding or soreness that didn't go I would be straight to the docs.Well done for the sexual health check.That would be daunting for me.

Hi, fellow sore bum sufferer here! You have my sympathy and well done getting it all checked out, an embarrassing situation I know but you get used to it. Over a year now for me but mine is a fissure, feels much better a year later after numerous creams and eating very well etc. Shortly having Botox injections in bum to hopefully heal it for good. I don’t really get pain in going to loo but a sore feeling and slight ache all day really. Some days I do t suffer much at all. But to begin with I was sore, felt swollen etc. Hope you get some answers and feel better soon.

Have you ever been tested for coeliac disease? Or tried an elimination diet to rule out food intolerances?

When I had a sore arse for months, wind and tummy troubles it turned out that gluten was the culprit.

I've mostly had an easier week of things. Most of the time I'm not aware of very much discomfort or soreness at all, but some days I still get the 'dry anus' feeling (sorry, far tmi I know!) By that I mean that under normal circumstances the external sphincter feels moist and soft, whereas this trouble has often left me feeling like that part of my anatomy is a bit dried out. And sometimes when I sit on the loo, that same bit feels a bit 'achey', but that usually calms down after I've finished. I'm still not putting anything on it, just wiping with a dampened cotton pad after using the loo roll to clean up. The internal pressure is mostly unnoticeable. No itchiness, but occasional soreness at the old 'entrance' that doesn't last long. By now I'm almost convinced it's not threadworm, but who really knows, so many people I've read about on the internet never get a definitive threadworm diagnosis and feel that the treatment isn't effective for them.

The wind is also much better, which is a relief! Still curious to know what the colorectal appointment might reveal later this month though, as I'm still thinking it might be a small fissure that has only calmed down because of my diet and daily sitz baths. As I type I'm feeling slight rectal pain, and since they say internal hemmorrhoids aren't painful I'd be surprised if there isn't a tiny tear in there somewhere. We'll see.

MissTulipan I read your amazing thread and learned so much from your struggle and some of the other posters on there. I'm so sorry that you've been though so much with your fissure and really hope the Botox treatment will finally put an end to your suffering with all this.

There's somethng a bit odd about coming on here and talking about my arse, lol. But being a gay man I feel a little less creepy for doing it on a forum mostly frequented by ladies! Big hugs to you all Flowers
