Repeat Uterine Ablation or hysterectomy???

Hi, Around 4 years ago I had ablation due to v heavy periods and pelvic pain. I'm on Warfarin due to previous dvts , have had cardiac surgery (sternotomy) which resulted in paralyzed right diaphragm. I've also had 3 c sections, surgery for severe post partum haemmorage, and surgery for abdominal adhesions. After the ablation my periods stopped for around 2 years. However over last 2 years they have got increasingly bad and am now passing large blood clots with severe pelvic pain (sorry tmi)! I am due to see my gyne soon but after surgery to remove adhesions he said next step is an hysterectomy, but due to complicated medical history, would need to think long and hard about decision ( am on Warfarin and have paralyzed diaphragm which complicates surgery). Has anyone had repeat ablation with successful outcome or is hysterectomy my only option to stop this monthly nightmare?? I'm 46 yrs old. Sorry for long complicated post!

DIdnt want to read and run. Sounds horrendous val I don’t have any experience of ablation (yet) but have stage 4 endo and adenomyosis so ablation may be on the horizon and I have thought about the options, so happy to throw my tuppence worth in to keep you company!

Can you ask the Gynae what the outcome would be for a second ablation? I thought the ablation was supposed to be permanent, did some lining get left behind after the last one or is there something else going on? Do you have a diagnosis of endo or adenomyosis?

I’d be pushing for a fuller diagnosis before you decide. Are they talking about a total hysterectomy with an oophretomy (sp?) as well? Do you know when your mum went through the menopause, that’s usually a good yard stick to predict roughly when to expect it yourself (or so I’ve been told!) If you thought you only had a couple of years more periods to deal with would another ablation be a stop gap till you got there rather than major surgery given all the other complicating factors there are with your medical history?

No real advice. Your medical history makes things much more complicated.

I had an ablation in 2016 but it didn't work, Hysterectomy last June which has honestly been the best thing I've ever done. But my situation was different just fibroids causing my heavy and constant bleeding. I'm 49.

Hope your gynaecologist can advise.

Thank you for your replies.Badweek, I was also told that ablation would be permanent so will be questioning my Gyne about that. My mum died when she was 32 so unfortunately no menopause yardstick there. I was also thinking that if I got 2/3 years out of next ablation that would bring me closer to Menopause. I have had surgery to remove adhesions and Gyne said only way to remove them all is by hysterectomy with ovarie as they were v extensive and they are contributing to pelvic pain also.. Thanks BG for your reply and glad to hear positive outcome from your hysterectomy. I suppose I just don't want to bring more complications on myself as most other surgeries/ procedures have not gone as planned for me. But on other hand would love to be free of this monthly ordeal!

val im sorry you lost our mum so young.
Make sure when you see your gynae that you’ve written a list o question you want answered before you leave. It’s all too easy to get flustered and swept along in consultations by what they want to tell you rather than what you want to know. Take someone with you if you can as well so there’s a second pair of ears to remember everything afterwards and tonprompy you if you’ve forgotten to ask something you wanted to.
