Aching bones, tired, bleeding, hot at night etc

I'm mid 40s and for the past few months I'm being feeling unwell almost constantly - not horrendously unwell but enough to make doing anything more than staying in bed with limited activities a struggle.

My bones and muscles have been aching all the time all over, more bones than muscles if that's possible. I keep feeling hot and last night I woke up with the bottom half of my legs sweating and I never sweat. Constantly tired, can get dizzy especially during periods (which I'm still having but start off heavy bleeding then half water/urine - half blood). Lots of urinary issues to point have to work from home. Irregular bright red bleeding from both sources though tests done and a fibroid found and know I have haemorrhoids. Tiredness may just be lack of sleep. Going through hospital tests but they aren't showing much and it's 2 months between each one.

Husband says I need more exercise - I'm thin but don't exercise as I feel weak though it maybe a vicous circle. I do have anxiety but am happy. Any ideas what this could be / what I can do? Not getting very far with doctors / NHS but can go private though never done that so not sure how you do it. Thanks very much. I've started taking a multi vitamin with vitamin D and B vitamins in and seems to be improving things a little.

Amongst all the tests so far, I didn’t spot mention of a full blood count. Has one been done?

Thanks very much - no not had any blood tests for about 3 years and then all came back normal except low folate though iron wasn't tested. I do think I need some blood tests.

Gynaecology appointment at the private hospital on the NHS was really good, he couldn't examine me as it was my period and says urine symptoms will have to be dealt with my urology but arranged a scan to see the size of the fibroid on 17th and a further appointment with him on 6/2 when I shouldn't have period so can examine me. He did say he doubts it will be a fistula but I have an appointment at urology on 24/1 and can ask there. The urine does only appear to be leaking every couple of hours or so so maybe the kegels are helping.

I still have the aches and pains all the time and have been really dizzy this past couple of weeks - first week was my period so that's normal for me but this week I've been so dizzy and only able to wfh with lots of sleeping inbetween. My vision is going blurry / numbers sometimes jumping round on page but this maybe dizziness. Hard to describe dizziness - I feel very unstable on my feet like my head is going round felt like my head was going to fall off at one point. My husband reckons it's tiredness and I am not getting enough sleep thanks DD but I do catch up in the day and I don't think lack of sleep makes you that dizzy. Maybe it's anxiety but really not sure. I wondered about anaemia but my husband says I can't have that as I'm not pale but don't know if you have to be pale. He did his you need to get out of bed more helpful advice but I can go out if accompanied but quickly start feeling ill. Nothing noticeable wrong with ears, very mild cold but really nothing. Don't think I have a temperature but did get antibiotics before Christmas which seemed to help with uti symptoms. Work are not going to be happy with me. Thanks for any help. I have got more vitamin D tablets. I did try ones with iron in but they made me ill, maybe the liquid is easier to tolerate.

Been for ultrasound today transabdominal and t/v, took 30 mins and didn't give results.

I did ask about my large fibroid which I had 4 years ago and she said I now have two small fibroids. Confused

I googled and low estrogen causes a fibroid to shrink, also seems to give bone problems and tiredness so wondering if I have that but then I have a second fibroid but guess it could have appeared 3.5 years ago grown then both shrunk. Does add up with symptoms except urine ones. Lady said the fibroids would add pressure but shouldn't cause all urine issues.

Do you bruise easily?

I do think you need bloods done, because you have tiredness/exhaustion. sweats (sometimes excessive), sore bones/
joints and unusual bleeding.

Re bruising easily not sure - I do get bruises I have no idea where they've come from but I also get jumped on by kids. Currently I only had 1 or 2 small 1 inch light bruises on the whole of me so I wouldn't say I do. Having said that I went to a test and had a large bruise and the lady said where did you get that from and I said I don't know, maybe kids jumping on me (I really don't know) and the lady said I must bruise easily like her but not sure my kids are a reasonable size though thin.

I think I need a hormone test, iron test, maybe thyroid don't think it's that but best to test, maybe diabetes don't think I have that but runs in family. I wouldn't be surprised if bloods don't show much but even so that tells me things. I think work have one of those full checks up this year but not sure when.

Also, would you say the UTI is slow/difficult to clear?

Thanks - utis tend to clear with 3 day antibiotics but I do seem to get them quite a bit or at least symptoms like a uti, burning, pain, feeling hot etc sometimes I'm testing negative (about 50% of time) but each time I've had antibiotics, 2 or 3 times in past year I've felt better.

Saw urology today.

Said he would refer me to physio which would try for 6 months before surgery and some new meds via GP. The meds help with urge which he says is only a very small part of what I have so not sure if it will make a difference. Its almost all stress. Said urodynamics saw bad leakage quickly and constant and they couldn't fill bladder more than 200mls where normally would fill to 400ml.

I then asked him if a fistula was possible and explained about getting urine through the vagina and it's possibly all coming through vagina. He got quite excited by that and said although I have no indicators for a fistula its always possible and symptoms match it very well and said first doctor has missed this. He said this changes everything though will still do physio and meds but will run two tests one under local which takes 5 mins for large fistulas, if that's clear will run a second test I presume that maybe under general. If either find a fistula then will then run further tests and I think surgery. So at least it's quite thorough checks. I was expecting him to not take the fistula idea seriously as the gynae doctor said I wouldn't have it as no risk factors but he seems to think it maybe it. Quite amusing it got him so excited though.

Spoke to GP as getting worried this is cancer and he agreed they are likely to find a vaginal fistula. He had results of second scan which just showed enlarged bladder wall and 2 small fibroids rest looks normal. He said that's encouraging for it not being cancer but agreed my symptoms are odd and I need to do cancer tests. He's leaving a form for me to do blood tests at the surgery.

Got some results back now - low iron though only borderline low and kidney function is abnormal.

Thyroid - normal, blood sugar - normal, bones - normal, calcium - normal, cholesterol - normal

Lost and need redoing FBC and probably lost FSH/LH and vit D. Doctor expecting FBC to come back low. Says kidney needs testing every 3 months and lots more tests. Need to take iron supplement but will try liquid as the tablets make me ill.

The liquid iron supplement Feroglobin seems to have stopped the bone pain though there's other things in it so may not be in the iron, improved quite quickly though have been taking vitamin D as well but that alone didn't help bones.

Cystoscopy showed no signs of cancer or a fistula though can miss small ones but that doctor thinks its overactive bladder.

Had a hysteroscopy/ biopsy and fibroid resection today - will be interesting to see what difference that makes, fibroid wasn't that large though, must have shrunk, only 2cms and second one is in the muscle so they can't touch.
