Hi , we’ve just qualified for my mothers continuing health care however they haven’t told me how much ? I know that each case is different but what’s the max as I’m looking at nursing homes

Are they fully funding, a percentage or FNC? In my area they have preferred places, but willing to consider most options (their preferred places are usually the more expensive homes)

Is your mum currently in a home? They should actually give you a number where you can ask this kind of stuff.

My mother’s needs are very complex she will need both nursing & medical care
She’s currently in hospital , I would have thought that the chc would pick the cheapest

Nursing and medical- never heard of it, what exactly do you mean, there will be only nurses (unless it’s a specialist hospital that takes long term placements).

Where’s your mum at the moment?

Often what you think is really complex isn’t actually that complex to those who work in the area.

They do they don’t always go with the cheapest

For example
Home a, qualifed etc, but they know they aren’t good with dealing with difficult behaviour caused by dementia, cheapest

Home b, also qualifed etc, great at dealing with complex behaviour, more expensive

They know if someone with complex behaviour (or seizures, or medication, nursing needs, pressure sores, peg feeds, colorosmy bags, catheters, oxygen or whatever else the main problem is) goes into home a then there will be problems and will most likely have to move, so in some cases with just recommend Home b.

Sorry have just reread that she’s in hospital, ask the nurses that completed the assessment what the funding breakdown is or if it’s fully funded.

Ask for homes they contract with and which would suit your mums needs.

Ask how long till it’s reviewed and when it’s reviewed could she move to a lower level funding, if so find out what the financial impact would be on your mums finances (often people leave hospital fully funded, but can then at review be moved to partly funded and family/possibly social services will have to pick up the bill.

I know one person in a specialist younger person mental health, who is funded at £2300 per week.

At the lower end of the scale funded nursing care is £150 per week.

Nursing homes are usually around £800 per week where I live.

I work in CHC , is your mother fully CHC funded or is it a 'funded out of hospital' pathway? The latter is often mistaken for full CHC but is definitely not.

I am assuming it's the former. the CHC care coordinator will meet and a care plan will be agreed. If she is in hospital now they will most likely consult you and your family on a suitable Nursing home that meets her needs and your's as a family for visiting. If they have spoken about a Personal Health Budget PHB , that will be agreed by the care coordinator and commissioning a budget and all it's details..such as what it can be used for.
The bottom line is her health needs must be met .Nursing homes will often make an agreement with the CCG on a fee.

My mum is fully funded ggirl , I have asked a few times about how much they are willing to contribute but they haven’t said
My mum is bed bound , double incontinent , suffering with delirium /dementia she will need round the clock care . The discharge nurse asked if I’d been given a list of care homes which I haven’t . I’ve basically looking myself & reading the ccq web site. I was told by social services that they are finding it difficult to find a nursing home that can accommodate my mums needs

If your mum is fully funded then social services should no longer be involved in arranging her care/finding care homes. It's the reponsibility of the NHS.

Go back to the nurse assessor who completed the decision support tool and find out what's going on. the reason you haven't been given a figure is because they will most likely fund the full amount of whichever nursing home can be found to accept her, unless you choose a super duper bells and whistles expensive one and there is a cheaper placement available that can meet her needs.

In my area you would have been sent an official letter re her eligibility for CHC funding , and told that a co-ordinator would be in touch to discuss and finalise the plan of care i.e. where would meet her needs.
There will be a number on the letter you can call for help.
Most nursing homes would be able to deal with your mother's needs , if she is mobile she will need a home that has enough staff and is locked , a dementia specialist home.

Social services shouldn't be involved unless she is dual funded.
Have you not had a letter informing you of the eligibility , it will detail the plan and have a phone number for you to call.

It sounds as if your mum will need a nursing home, what area are you looking at? The nursing home manager will need confirmation that CHC funding has been agreed and is in place before they area able to accept your mum. Carehome managers usually go to the hospital to visit and assess potential new residents to make sure they can meet their needs and that the family and patient are happy. I don't know why ss are having trouble finding a suitable home, maybe there are no vacancies at the moment. As others have said you need the CHC funding agreement in writing.

Yes we have received the CHC in writing and she will be fully funded . I will be visiting my mum in hospital tomorrow and find out what’s going on . Its Such a stressful time
